Rewrite md5.ts with proper TypeScript types

Replace eslint-disable blanket with fully typed implementation:
- Add return types to all functions
- Use number[] with fill(0) instead of sparse Array()
- Add xi() helper to safely index number[] under noUncheckedIndexedAccess
- Replace substr with slice
