0.2 What ocr-v2 is

A structural redesign of PaperForge's OCR pipeline. The original pipeline committed to semantic roles too early (raw OCR labels -> guess -> rescue -> render). ocr-v2 replaces this with anchor-first parsing: structural signatures -> stable anchors -> zone inference -> late role resolution -> figure/table validation -> render.

OCR-v2 readiness-gate work is complete. Post-readiness rebuild hardening is now the active queue.

0.4 Current OCR Slice

The first post-readiness rebuild-hardening batch is complete. The current slice is now split into two new design threads:

These are intentionally separate so table-local ownership and figure-group growth can be verified independently.


1. Architecture Overview

1.1 The problem (pre-v2)

raw_label/text -> final role -> normalize -> rescue -> demote/promote -> attach -> render

Early role guesses caused: title/author/footnote misclassification, body paragraphs typed as references, reference continuations leaking into body, left/right column interleave bugs, duplicate figure captions.

1.2 The solution (v2 target)

raw observations -> structural signatures -> stable anchors/families -> zone inference -> role resolution -> figure/table validation -> render + health

1.3 Core principles

  1. seed_role is a proposal, never a final role -- assign_block_role() proposes; normalize_document_structure() decides
  2. VERIFY_REQUIRED roles (paper_title, authors, section_heading, reference_item, figure_caption, etc.) must have role_verification_status == "ACCEPT" with non-empty role_source and role_evidence
  3. Zone != Role -- being in the body reading environment does not imply body_paragraph
  4. Reference tail first -- reference sections protected from tail contamination
  5. Frontmatter source-backed -- OCR localizes but must not invent/discard canonical frontmatter

2. Development Phases (Timeline)

Phase 1: Artifact Foundation (earliest commits)

  • What: Reader-facing figure output separated from body prose.
  • Why: Figures must be reader-visible without polluting body flow.

Phase 8: Verified Structural Role Gate (2026-06-11)

  • docs: document OCR structural role gate (f281cbe)
  • feat: merge OCR role gate into health (39a69f9)
  • feat: install OCR verified role gate
  • feat: add OCR role gate context adapters
  • feat: verify OCR reference zone from document artifacts
  • What: Installed a document-level gate that prevents unverified high-risk roles from rendering. Uses VERIFY_REQUIRED set with role_verification_status, role_source, role_evidence.
  • Why: The original spec was being bypassed -- roles were being assigned and consumed without verification. The gate enforces the anchor-first contract.

Phase 10: Production-Path Root-Cause Remediation (2026-06-13, LATEST)

Trigger: Real-paper regression tests (CAQNW9Q2, DWQQK2YB) surfaced systematic gaps. Instead of patching symptoms, each fix targeted the production path upstream.

10a. Test Infrastructure

  • test: add real-paper trace vs expectations regression harness with fixtures (e055f6f)
  • block_trace.csv copied from vault into test fixtures for self-contained tests
  • expectations.json uses only real pipeline roles, updated as fixes land
  • Why: Needed ground-truth gap measurement before making production changes.

10b. Frontmatter Anchor Bridge (RC1)

  • fix: add ocr_page to source-backed frontmatter anchors (fe73c3f)
  • fix: bridge frontmatter anchors, add zone fallback... (5a72315)
  • Root cause: _build_source_frontmatter_anchor_ids read from source_frontmatter_anchor_ids but anchors were stored under source_frontmatter_anchors (different attribute name). paper_title/authors always HELD.
  • Fix: Bridge function now reads both attribute shapes. Added ocr_page to writer for page-qualified IDs.

10c. Figure Caption Gate (RC2)

  • fix: preserve figure_caption as candidate and extend reference item continuations (3b130a5)
  • test: add gate handler tests for figure_caption candidate preservation (417b5b9)
  • Root cause: figure_caption in VERIFY_REQUIRED but no handler in resolve_verified_role. Always HELD → unknown_structural.
  • Fix: Added figure_caption handler: when accepted_caption_block_ids empty, returns CANDIDATE with figure_caption_candidate (preserved for figure_inventory). Does not fake ACCEPT.

10d. unassigned Role Bug (the unifying discovery)

  • fix: use seed_role in zone fallback when role is unassigned (02cca45)
  • fix: handle unassigned role in zone infer_zones and frontmatter boundary detection (2ceaa2e)
  • Root cause: Zone inference and boundary detection run BEFORE structural gate. All blocks have role="unassigned" at that point. Code pattern role = block.get("role") or block.get("seed_role") failed because "unassigned" is truthy.
  • Fix: Added explicit if role == "unassigned": role = block.get("seed_role") in zone fallback, page-1 boundary scan, and post-reference backmatter detection.
  • Impact: DW empty zones 72→5, CAQ body_zone +5, page 1 frontmatter boundary working.

10e. Figure Pipeline Rescue

  • fix: relax figure caption narrative prose filter and add sequential matching fallback for cross-page captions (c407ca9)
  • fix: merge adjacent heading blocks at seed level, fix sequential match asset page (2364fb9)
  • Root cause 1: _is_body_mention rejected figure captions with figure numbers as body prose.
  • Root cause 2: Spatial matching required same-page legend+asset; sequential fallback added for cross-page captions.
  • Root cause 3: Sequential match used caption's page for crop, not asset's page.
  • Impact: DW 0→4 matched figures, CAQ 1→3. All EXACT_MATCH.

10f. Fulltext Rendering Cleanup

  • fix: remove internal status labels from reader figure card output (3c9d4f3)
  • fix: suppress reader figure cards when embed covers them (a26de38)
  • fix: remove blockquote prefix from abstract, fix backmatter_start detection (f522602)
  • fix: exclude bullet-prefixed highlight text from abstract rendering (06549fd)
  • Impact: EXACT_MATCH/LEGEND_ONLY status labels removed from figure cards. Figure cards suppressed when embed note covers them. Abstract rendered as plain text. Highlight bullets excluded from abstract.

10g. Backmatter Boundary Detection

  • fix: merge adjacent heading raw blocks before seed role assignment (8e9ec5a)
  • fix: handle unassigned role in zone infer_zones... (partial)
  • fix: use max(backward_start, references_start) for spread_end (f12f20c)
  • Root cause: _detect_backward_backmatter_start scanned backward for role matches but roles were "unassigned". Used dense_refs check that triggered on biography pages (misclassified as reference_items).
  • Fix: Added seed_role checks. Changed from early-return to min-page tracking. Added backmatter_heading_candidate to recognized roles. Requires reference_heading on same page for dense_refs. spread_end uses max of backward_start and references_start.
  • Impact: backmatter_start 33→25. Conflict of Interest + Acknowledgments promoted to backmatter_heading. Reading order fixed.

10h. Heading Merge (Adjacent OCR-Line-Wrap Prevention)

  • fix: merge adjacent heading raw blocks before seed role assignment (8e9ec5a)
  • Root cause: OCR splits long headings across lines; second half gets text raw_label instead of paragraph_title.
  • Fix: Before seed role assignment, merge adjacent paragraph_title blocks on same page + column with ≤30px vertical gap.
  • Constraint: Only ≤30px gap (same heading, different line). Does NOT merge separate headings at different y-positions. Column-aware to prevent cross-column merge.
  • Impact: CAQ "Increasing the accuracy and reproducibility in standard radiography" now single heading.

Phase 11: Structural Gate Anchor & Author Matching Fixes (2026-06-14)

Status: Committed on ocr-v2 branch. 5 files modified, 280 tests pass.

11a. Source Anchor Bridge to normalize_document_structure

  • Files: ocr_blocks.py (+18/-12), ocr_document.py (+5/-2)
  • Root cause: source_frontmatter_anchors built on OLD DocumentStructure, then normalize_document_structure() created a NEW one -- gate ran with empty anchors.
  • Fix: Build anchors BEFORE calling normalize, pass via source_frontmatter_anchors param.
  • Impact: TSCKAVIS doc_title -> paper_title ACCEPT, authors -> authors ACCEPT.

11b. Table Caption Gate Handler

  • File: ocr_structural_gate.py (+18 lines)
  • Root cause: table_caption in VERIFY_REQUIRED but no handler in resolve_verified_role.
  • Fix: Added table_caption handler mirroring figure_caption.

11c. Author Matching Overhaul

  • File: ocr_metadata.py (+43 lines)
  • Root causes: & in author text stripped to whitespace but not split; source metadata abbreviated names vs OCR full names; OCR merged trailing labels.
  • Fix: Strip trailing labels, normalize & to and, subset check, initial matching, partial initial matches.

11d. Frontmatter Noise Override

  • File: ocr_structural_gate.py (+1 line)
  • Root cause: frontmatter_noise in _SAFE_PRESERVED_ROLES preserved it even when seed_role=paper_title.
  • Fix: Added override condition for frontmatter_noise when seed_role is VERIFY_REQUIRED.

11e. Source Anchor Override for Unlabeled Authors

  • File: ocr_structural_gate.py (+11 lines)
  • Root cause: CAQNW9Q2 author block had seed_role=unknown_structural (OCR missed label). Gate only checked author anchors when seed_role == "authors".
  • Fix: Added source-anchor override loop for authors regardless of seed_role.

11f. formal-library.json Author Enrichment

  • File: ocr_rebuild.py (+17 lines)
  • Root cause: source_metadata.json only got first author from meta.json.
  • Fix: Fallback to formal-library.json index for full author lists.

11g. Heading Merge Vertical Gap Constraint

  • File: ocr_blocks.py (+10/-2 lines)
  • Root cause: .endswith((".", "?", "!")) too weak to prevent cross-heading merge.
  • Fix: Replaced with _vertical_gap() <= 30 (30px = same heading, different OCR line).

Phase 13: Final Gap Closure Round (2026-06-14)

Status: Tasks 1-9 complete + source-anchor page-scope fix. Traces regenerated.

13a. Real-Paper Trace Regeneration

  • Regenerated block_trace.csv for DWQQK2YB (287 blocks) and CAQNW9Q2 (153 blocks) from vault raw blocks.
  • Traces reflect full pipeline through Phase 11 fixes.

13d. Remaining Known Bugs

  1. DW preproof frontmatter: Title/authors/PII on page 1 still suppressed by preproof cover zone. Need seed-role rescue for preproof pages.
  2. DW biography page mismatch: Expectations list pages 33-34 but biographies actually span pages 32-34. Update expectations.
  3. CAQ page 7 zone conflict: ref_start=7 blocks Conclusion from body_zone. Need block-level vertical split on same page.
  4. CAQ correspondence footnote: Expected frontmatter_support but classified as frontmatter_noise.

13e. Render Layer & Boundary Protection Fixes (2026-06-14)

8. 2026-06-16: Gold Fixture Expansion + Pipeline Bug Fixes + Figure Rework

Session: Full-day debugging and repair session across 8 gold papers. Key deliverables: 98 bug annotations across expectations, 8 pipeline fixes, root cause analysis.

8.1 Gold Expectations — Comprehensive Block-Level Audit

All 8 gold papers (2GN9LMCW, 6FGDBFQN, A8E7SRVS, CAQNW9Q2, DWQQK2YB, K7R8PEKW, SAN9AYVR, TSCKAVIS) now have expected_bugs arrays and per-page assertions following the CAQNW9Q2/DWQQK2YB pattern.

Pattern:

  • Every significant block gets an assertion: text_contains + expected_role + expected_zone
  • Blocks where pipeline output differs have "notes": "BUG: Real: X. Should be Y."
  • Document-level expected_bugs array catalogs all known issues with fix guidance
  • Vision agents cross-verified some papers against annotated page images

8.4 Figure Processing — Critical Handoff

8.4.1 What Changed

Three layers of figure fixes were applied:

Layer 1 — Caption Binding (ocr_roles.py L1180+): vision_footnote blocks adjacent to existing figure_caption/figure_caption_candidate blocks (≤200px vertical gap) are promoted to figure_caption_candidate. Fixes panel sub-captions like "B, Sagittal oblique image..." being classified as footnote.

Layer 2 — Figure Matching (ocr_scores.py:score_figure_match L109+, ocr_figures.py:_looks_like_inline_figure_mention L67+):

  • _looks_like_inline_figure_mention: text starting with "Figure N." / "Fig. N." → NEVER flagged as body mention. Fixes long multi-panel captions being penalized for containing verbs like "illustrates", "shows".
  • score_figure_match: permissive gate — same-page + figure_number OR pipeline-labeled figure_caption → direct matched (score=0.75). Falls back to same-page with score≥0.25.

Layer 3 — Asset Expansion (ocr_figures.py L793+): After a legend matches a candidate group, ALL remaining unmatched media assets on the same page are absorbed into matched_assets. Ensures composite figure includes every panel, not just one group.

Layer 4 — Composite Region Assembly (ocr.py L1184): Removed not text_ids requirement in compute_precaption_composite_regions. Previously: composite regions required at least one text block inside the image area → rejected pages with pure image layouts (like K7R8PEKW pages 2, 5, 11). Now: only requires ≥1 media block + ≥2 total blocks.

8.4.2 Current State

  • Fulltext embeds: Figure 1-4 all render as ![[render/figures/figure_00N.md]] (not blockquotes).
  • Composite regions: compute_precaption_composite_regions correctly includes ALL image blocks for all figure pages.
  • Figure JPGs: NOT regenerated — rebuild function (run_derived_rebuild_for_keys) does NOT regenerate figure images. Figure JPGs are produced during the initial OCR run via ocr.py lines 1540-1556 using crop_block_asset(). ⚠️

8.4.3 Remaining Work for Next AI

A. Regenerate figure images:

  • Run full OCR pipeline (not just rebuild) for ALL 8 gold papers
  • The composite JPGs at PaperForge/ocr/<paper>/assets/figures/figure_00N.jpg should now be correct composite images containing all panels
  • Current cached JPGs may still be from pre-fix runs

B. Verify panel completeness visually:

  • Figure 1 → should show panels a-h (not just e, f, h)
  • Figure 3 → should show all panels (not just c, h, i)
  • Challenge: K7R8PEKW has complex multi-panel figures with irregular layouts

C. Figure grouping improvements (if needed):

  • Current _build_candidate_figure_groups_from_assets only handles same-row pairs/triples
  • Does NOT handle vertical multi-panel layouts (stacked panels in same column)
  • Expand to support vertical stack groups: same x-center ±40px, vertical gap ≤80px, consecutive in reading order

D. Candidate group suppress audit:

  • The single_asset suppress logic (F5 earlier) removes single-asset groups covered by multi-asset groups
  • Verify across all 8 papers that this doesn't accidentally suppress valid single-panel figures

8.5 K7R8PEKW Before/After

Headings:

  • Before: 10 blocks, "3. ... 3.1." merged, section 7 suppressed
  • After: 16 blocks, all split, correct hierarchy (#########)

Figures:

  • Before: 1 embed (figure_002), 3 blockquotes
  • After: 4 embeds (figure_001-004), all correctly matched

Remaining known issues:

  • Section 7 heading rescued from unknown_structural (works now)
  • Frontmatter author footnotes still render as body text
  • Body text fragmentation across pages (not fixed — user accepted as acceptable: blocks present and in order)
  • Figure JPGs need full OCR re-run to reflect composite region fix

8.6 Parked Hard Case

  • Parked hard case: pages that contain two adjacent formal figure captions (for example, Figure 2 and Figure 3) whose visual regions are close enough that auto-separation is risky. Current policy is conservative: do not cross caption ownership boundaries automatically; prefer unresolved over wrong merge. Requires a later dedicated partitioning design.
  • New scoped fallback family: same-page narrow-caption sidecar layouts. Trigger only after existing legend recognition succeeds and only when captions are narrow, same-column, and the normal match path is ambiguous. Do not use this path for full-width captions, backmatter figure compilations, or mixed table/figure pages.

8.7 K7R8PEKW Tail Rendering Fix (2026-06-17)

Problem: K7R8PEKW page 16 rendered fulltext had references out of numerical order, backmatter mixed with refs, and keywords content orphaned after refs.

Root cause: Two issues compounded:

  1. No reference heading block — all ref items scattered through body_pool → orphan_blocks.
  2. Forced backmatter section grouping_normalize_backmatter_roles_after_boundary and _reorder_tail_run converted section headings (Keywords → sub_subsection_heading) into backmatter sections, then grouped them by Y position. On two-column tail pages without a ref heading, this overrode the natural column-sorted reading order.

Fix:

  • _reorder_tail_run: Added skip_section_grouping param. Pages with reference_item but no reference_heading bypass the section-grouping Phase 1-5 logic entirely — blocks are emitted in column-sorted order with reference items grouped at the end.
  • _order_tail_blocks: Detection of per-page ref-item / ref-heading presence to set the flag.
  • _normalize_backmatter_roles_after_boundary: Removed forced body_paragraph → backmatter_body conversion (not needed — body paragraphs attach naturally via _find_owning_heading in Phase 1).
  • Synthetic ref section fix (Phase 2.5 in _reorder_tail_run) kept: creates {"heading": None, "bodies": []} when ref_items exist but no ref_heading to prevent scattering.

Result: Page 16 now renders in correct column-sorted order:

Tests: 80 figure tests pass, 7 pre-existing failures unchanged. K7 not in failure list.

8.8 Layout-First OCR Phase 1 Pass (2026-06-17)

Problem: The next OCR cleanup phase needed to be layout-first and low-regression after the PDF text fallback work. The specific Phase 1 targets were: stop dropping plausible table assets too early, stop blanket-downgrading strong formal captions to _candidate, and lock regression coverage around those behaviors.

Root cause:

  • paperforge/worker/ocr_tables.py only let media_asset enter table matching when raw_label == "table", so plausible table images labeled as generic media never even reached caption matching.
  • paperforge/worker/ocr_document.py blanket-downgraded figure/table caption seeds whenever accepted artifact ids were empty, even for strong numbered display legends.
  • The branch already had working panel-label and margin-side protections, but there was no dedicated regression file proving those layout-first behaviors stay intact.

Fix:

  • Added tests/test_ocr_layout_first_regressions.py with focused checks for panel labels, margin-side notices, table admission from media_asset, and selective caption-seed retention.
  • Relaxed the table inventory gate so large enough media_asset blocks can participate in table matching.
  • Added _should_keep_formal_caption_seed() and used it to avoid blanket _candidate downgrade for strong numbered display legends.
  • Rebuilt the 6 affected papers only: 6FGDBFQN, A8E7SRVS, CAQNW9Q2, K7R8PEKW, SAN9AYVR, TSCKAVIS.
  • Re-ran diff_audit.py for those 6 papers without re-running PaddleOCR and without wiping audit/.

Result (Round 1 — table + document gate fixes):

  • Code paths are now covered by targeted regression tests. Post-rebuild mismatch totals did not materially move: figure_caption_candidate 129 -> 129, media_asset -> body_paragraph 42 -> 42, unknown_structural 53 -> 53.

Result (Round 2 — lowercase panel-label fix):

  • Extended _PANEL_LABEL_PATTERN from [A-Z] to [A-Za-z] to catch lowercase sub-panel labels (a), (b), etc.
  • figure_caption_candidate mismatches: 129 -> 79 (50)
  • figure_inner_text vs figure_caption_candidate: 54 -> 7 (47, recovered by the pattern fix)
  • Remaining 7 are non-letter sub-panel content (concentration labels, named panels)
  • SAN9AYVR mismatches dropped 117 -> 91; DWQQK2YB 38 -> 35

Test status:

  • Red/green proof: pytest tests/test_ocr_layout_first_regressions.py -v --tb=short initially failed on 2 tests, then passed after the patch.
  • Fresh targeted verification: pytest tests/test_ocr_layout_first_regressions.py tests/test_ocr_roles.py -v --tb=short -> 72 passed.

9.7 Task 6 — P0-P2 Layout Close-Out: Preproof Frontmatter + Margin-Band Noise + Figure Inner Labels (2026-06-18)

Problem: Three P0-P2 layout gaps remained after boundary close-out:

  1. P0: Preproof page-1 drop removed all frontmatter blocks — infer_zones() and assign_block_role() used hardcoded page_num == 1, so title/authors on the first surviving page (page 2 of source, page 1 of blocks) were treated as body blocks or dropped
  2. P1: Margin-band publisher blocks ("Downloaded from ...", "For personal use only") with width ≥ 10% slipped through _looks_like_margin_band_noise() — no confirmatory watermark text fallback existed
  3. P2: Figure inner labels adjacent to figures were not identified when they contained mixed alphanumeric content (e.g., "Table 1." as inner text)

Root cause:

  • _first_surviving_page() didn't exist; all zone/role dispatch hardcoded page_num == 1
  • _looks_like_margin_band_noise width threshold was 10% with no text-based alternative; some publisher blocks are intentionally wider
  • _looks_like_figure_inner_label() required purely alphabetical tokens; "Table 1" / "Figure 2" caption-adjacent labels with alnum mixed tokens were missed

Fix:

  • Added _first_surviving_page() helper and _is_first_page_body_start()infer_zones() uses first-surviving-page as frontmatter origin; body-block eligibility checks it; boundary bands scope to it
  • Updated _detect_frontmatter_zone() to remove page-num guard (caller decides which pages to check)
  • Added _has_confirmatory_watermark_text() — checks "downloaded from" / "for personal use only" — used as alternative gate in _looks_like_margin_band_noise()

Root cause: project/current/ and PROJECT-MANAGEMENT.md were not fully reconciled after the P0-P2 close-out session.

Fix: Rewrote the active close-out priority and remaining-issues files so only real unresolved DWQQK2YB support-routing and ownership issues remain active; completeness-check stays as the next planned slice.

Result: Later implementation steps no longer inherit already-fixed P0-P2 failures as active blockers.


10. 2026-06-22: Cross-Page Caption Consumption Fix

10.1 Reader/Render Contract Root Cause (2026-06-22)

Problem:

  • Cross-page figure ownership for 2HEUD5P9 had been repaired at the inventory layer, but the reader/render chain still consumed caption blocks using the asset page instead of the true legend page.
  • This left a backend contract breach: cross-page matches could still leak the original caption on legend_page, or at minimum keep inconsistent caption-consumption metadata.

Root cause:

  • paperforge/worker/ocr_figures.py already emits cross-page matched figures with distinct page and legend_page fields.
  • paperforge/worker/ocr_figure_reader.py::_normalize_bucket() kept page, but did not preserve legend_page as first-class normalized data.
  • _materialize_reader_figure() then built consumed_caption_block_ids using normalized_item["page"], which is the asset page, not the caption page.
  • paperforge/worker/ocr_render.py correctly suppresses captions by (page, block_id), so once the reader handed it the wrong page, the contract was already broken upstream.

Fix:

  • Added _caption_consumption_page() in ocr_figure_reader.py.
  • _normalize_bucket() now preserves legend_page from strict inventory items.
  • All reader materialization paths that consume captions (matched_figures, grouped approximate, held/legend-only, unmatched legends) now prefer legend_page, falling back to page only when legend_page is absent.
  • Added regression coverage:
    • tests/test_ocr_figure_reader.py::test_reader_matched_cross_page_figure_consumes_caption_on_legend_page
    • tests/test_ocr_render.py::test_render_fulltext_markdown_suppresses_cross_page_caption_on_legend_page

Result:

  • Reader payload now records cross-page caption consumption on the real legend page.
  • Render path suppresses the original page-13 caption block while still allowing the figure object card to render normally.
  • This fixes the highest-value contract bug without expanding scope into orphan recomputation or broader fallback redesign.

Test status:

  • Reserved cross-page settlement still picks prior/next-page owners by order rather than true compatibility when multiple candidates exist.
  • Sidecar fallback still repartitions all page assets, not just unresolved/unowned ones.
  • Figure/table ownership still lacks a shared consumed registry for ambiguous image-like blocks.

Root-cause notes for the remaining issues:

  • unmatched_assets is rebuilt once immediately after primary cross-page settlement, but later sidecar / group-sequential / legacy sequential fallback passes still consume assets without a final orphan recomputation.
  • Reserved cross-page settlement currently chooses the first sorted compatible prior/next target rather than re-scoring multiple semantic-group candidates.
  • Sidecar fallback rebuilds bands from the whole page asset list instead of just still-unowned assets, so it can theoretically reassign already-owned media.
  • Figure and table matching still maintain separate ownership registries; image-like media_asset blocks can satisfy figure and table admission rules independently.

10.2 Final Orphan Recompute Fix (2026-06-22)

Problem:

  • unmatched_assets / orphan outputs were correct immediately after primary cross-page settlement, but became stale after later ownership passes.
  • This left a mismatch between actual used_asset_page_ids truth and the exported orphan inventory.

Root cause:

  • build_figure_inventory() rebuilt unmatched_assets once after Stage 1 cross-page settlement, then later sidecar / group-sequential / legacy sequential fallback passes continued consuming assets.
  • No final inventory-wide recomputation happened before returning figure_inventory.
  • As a result, assets already owned by late fallback passes could still remain in unmatched_assets and later be emitted as orphan outputs.

Fix:

  • Added _recompute_final_unmatched_assets() in paperforge/worker/ocr_figures.py.
  • The helper now computes final orphan truth from:
    • all original assets
    • final used_asset_page_ids
    • unresolved_clusters ownership
  • build_figure_inventory() now runs this recomputation once at the end of the matching/fallback pipeline, just before assembling the final inventory payload.
  • Updated regression expectation for cross-page asset ownership and added a new late-fallback recomputation test:
    • tests/test_ocr_figures.py::test_final_unmatched_assets_recomputed_after_late_fallback_match

Result:

  • Assets consumed by late fallback passes no longer remain in unmatched_assets.
  • Orphan output truth now matches final ownership truth rather than an intermediate pipeline snapshot.

Test status:

10.3 Cross-Page Figure Double-Emit Fix (SAN9AYVR Figure 24) (2026-06-22)

Problem:

  • In SAN9AYVR, Figure 24 was matched only once in inventory/reader structures, but fulltext.md embedded figure_024.md twice.
  • The duplicate appeared once on the asset page and once again on the legend page.

Root cause:

  • This was not a matching duplication bug.
  • figure_inventory.json contained one matched_figures entry for figure_024 and reader_figures.json contained one figure_024_reader entry.
  • The duplication happened in ocr_render.py because two independent render channels both emitted the same figure:
    • legacy figures_by_page emitted figure_024.md on asset page 50
    • reader-driven emission emitted the same embed again on legend page 51 via reader_figures_by_page
  • Render had no cross-surface dedup rule saying that if a reader figure already covers figure_024, the legacy matched_figures path must not also emit it.

Fix:

  • Added _reader_covered_figure_ids() in ocr_render.py.
  • render_fulltext_markdown() now computes the set of figure ids already covered by reader figures and excludes those ids from the legacy figures_by_page emission map.
  • Added regression coverage:
    • tests/test_ocr_render.py::test_render_fulltext_markdown_does_not_double_emit_cross_page_figure_embed

Result:

  • Cross-page figures no longer emit once from the legacy figure path and again from the reader path.
  • Rebuilding SAN9AYVR now leaves exactly one ![[render/figures/figure_024.md]] embed in fulltext.md.

Test status:

9.9 Task 2 — DWQQK2YB Support + Ownership Repair And Rebuild (2026-06-18)

Problem: After 9.7, DWQQK2YB still had one real frontmatter-support routing gap and one real same-page figure ownership gap; derived assets also lagged behind the repaired code path.

Root cause: early support routing still leaned on page-1/text-special-case logic, and same-page figure claiming treated broad page media clusters as one ownership pool.

Fix: promoted first-surviving-page frontmatter support lines using bounded support-text confirmation; constrained figure claiming with expansion bound in _expand_matched_assets_locally; rebuilt DWQQK2YB derived artifacts with scripts/dev/ocr_rebuild_paper.py.

Result: DW support blocks remain in frontmatter support flow (frontmatter_support: 2 in rebuild role distribution); figure ownership regression now verifies correct page assignments.

Problem: The branch no longer lacked a local close-out fix; it lacked one explicit readiness model. Active project truth files still mixed completed close-out language, deferred figure work, and broader generalization concerns without one formal execution thread.

Root cause: project/current/ and PROJECT-MANAGEMENT.md were updated incrementally across close-out sessions, but they were not yet rewritten around the approved readiness-gates framing.

Fix: wrote docs/superpowers/specs/2026-06-18-ocr-v2-readiness-gates-design.md and realigned the active project truth files so the branch now has one queue: Gate 1 completeness, Gate 2 figure ownership, Gate 3 ordering/boundary authority, Gate 4 layout coverage, then Gate 5 blind audit as the next-stage validation gate. Also removed the stale assumption that Gate 4 should be driven by a fixture-side coverage_manifest.json; the real-paper readiness ledger now needs to be defined against the live audit/ corpus.

Result: "state healthy" now has an explicit definition. The active queue is no longer described mainly as close-out or zone-boundary cleanup; it is described as a readiness-gates program with one authoritative next plan.

Test status:

Planning/docs only in this step.
No runtime verification claimed.

Problem: Group-first matching infrastructure already existed but DWQQK2YB Fig 3 on mixed-layout page remained ambiguous.

Result:

  • Existing group-first unit tests pass (infrastructure already in place)
  • Added test_dwqqk2yb_figure3_is_fully_owned_not_merely_captured as xfail with strict=True
  • Grouped-vs-single counters already present in health output

Problem: normalize_document_structure didn't use seed_role to enforce reference zone boundary on same-page mixed layouts.

Fix:

  • Added _same_page_reference_boundary_y() helper and _enforce_reference_boundary_from_structure() driver
  • Integrated call after _sanitize_reference_zone_boundary() in normalize flow
  • Added unit test for same-page body/reference boundary resolution

10.5 Task 5 — Gate 4: Layout-Coverage Formalization

Problem: Layout tags used generic labels (multi_column, single_column) instead of readiness-class taxonomy.

Fix:

  • Normalized audit/coverage_ledger.json to only use approved readiness-class tags (multi_panel, side_caption, same_page_ref_body_split, post_reference_biography, preproof_frontmatter, review_callout, special_structure)
  • Contract tests enforce named representatives per class

10.9 Task 7 — Readiness Review Hole Closure (2026-06-18)

Problem: Post-implementation review found that Gate 1 completeness helpers were only unit-tested seams, not runtime health inputs; missing-PDF baseline semantics were misleading; and two audited papers had no readiness-class tags.

Root cause: The readiness pass added helper functions and taxonomy scaffolding, but did not finish the production-health wiring or the full audit-ledger classification pass.

Fix: wired completeness summaries into build_ocr_health() so the runtime health path now emits page coverage, region-completeness summary, and rendered-gap counts when PDF-side hints are present; normalized rendered-gap comparison for whitespace/case drift; changed missing-PDF coverage ratio to None instead of fake 1.0; added a stronger audit contract requiring every audited paper to carry at least one layout tag; tagged K7R8PEKW and SAN9AYVR in audit/coverage_ledger.json; strengthened the Gate 3 boundary test with explicit zone assertions.

Result: The two review-critical holes are closed at the code/test level, and Gate 4 no longer leaves 25% of the audit corpus unclassified. Remaining readiness work still centers on Gate 2 figure ownership refinement.

10.10 Task 8 — Gate 2 DW Figure 3 Ownership Closure (2026-06-18)

Problem: Gate 2 still failed the strict DW Figure 3 regression because the caption lived on page 40 while its owned media lived on page 39, and the inventory path only tolerated that case as ambiguous/unresolved.

Root cause: build_figure_inventory() had no usable previous-page sequential fallback for a strong next-page caption, and when a later fallback did claim the asset, the stale ambiguous entry for the same legend was left behind.

Fix: added a focused figure unit test for previous-page sequential fallback, taught the sequential fallback to claim the nearest unclaimed asset from the immediately previous page when the numbered caption starts the next page, and removed stale ambiguous_figures entries when that fallback successfully matches the legend. Then promoted the DW strict regression from xfail to a normal passing test.

Result: DW Figure 3 is now strictly matched instead of "captured but ambiguous," closing the remaining Gate 2 readiness gap on the tracked hard case.

10.11 Task 9 — Broader Readiness Sweep After Gate 2 Closure (2026-06-18)

Problem: The targeted Gate 2 DW Figure 3 fix needed to be tested against the broader readiness sweep before the branch could claim all current gates were green.

Root cause: The Gate 2 patch introduced a broader previous-page sequential fallback rule. While it fixed the tracked DW regression, that rule also widened generic matching behavior beyond the older figure-suite assumptions.

Fix: Ran the broader readiness sweep across health, document, figure, real-paper regression, audit-contract, CLI/unit, and lint surfaces. Wrote the result back into project truth files instead of prematurely promoting Gate 2 to DONE.

Result: Gates 1, 3, and 4 remained green on the verified suites. Targeted DW Gate 2 regression initially turned green, but the first broader figure sweep exposed 7 matcher regressions, proving the new fallback rule was too broad.

10.12 Task 10 — Gate 2 Fallback Tightening With Layout Cross-Check (2026-06-18)

Problem: The first Gate 2 closure patch solved DW Figure 3 but made the generic figure matcher too permissive: sidecar fallback overreached, inline mentions were treated as formal legends, and same-page weak candidates were being forced through the sequential path.

Root cause: The new previous-page fallback lacked enough local layout cross-checks, and several existing generic matcher branches were still missing disambiguation guards (unnumbered sidecar legends, above-vs-below candidate ties, inline mention rejection, and caption-before-assets vs caption-after-assets partitioning).

Fix: tightened build_figure_inventory() in four places: (1) previous-page sequential fallback now requires a strong numbered next-page caption plus post-reference and page-edge geometry evidence, (2) same-page weak sequential fallback is no longer allowed to manufacture matches, (3) sidecar partition now uses a local layout-orientation cross-check to choose between caption-before-assets and caption-after-assets mapping, and (4) inline figure mentions plus close above/below asset ties are explicitly rejected/held instead of forced into matches.

Result: DW Figure 3 stays strictly matched, and the generic figure suite is green again. This closes Gate 2 on the currently tracked readiness suites instead of just on the one DW hard case.

10.13 Task 11 — Audit Corpus Diff + Canonical Roles + Structural Gate (2026-06-19)

Problem: The audit corpus diff showed 117 out of 1097 reviewed blocks as "wrong", mostly because (a) diff_audit.py used exact string matching without canonical role aliases, (b) block_review.jsonl truth roles used non-canonical names (media_assetfigure_asset, structural_noisenoise, etc.), and (c) strong table captions ("Table N." text) were stuck at table_caption_candidate because the structural gate required table inventory verification that never ran.

Root cause: Three independent issues:

  1. diff_audit.py compared pipe_role == truth_role without normalizing alias names — 31 of 43 "wrong" blocks in 6FGDBFQN were just alias mismatches, not pipeline errors.
  2. The structural gate's resolve_verified_role() had no fallback for table captions: blocks with seed_role=table_caption were always downgraded to table_caption_candidate when table inventory was empty.
  3. Audit truth had been written with role names that predated the pipeline's final canonical role taxonomy.

Fix:

  • diff_audit.py: added _CANONICAL_ROLE alias map and bidirectional canonicalization (_canonical(pipe) == _canonical(truth)); writes back normalized truth_role to block_review.jsonl.
  • ocr_structural_gate.py: added text-evidence fallback for table captions — if marker_type=table_number or zone=display_zone + text starts with "Table", accept without inventory (figure caption fallback skipped — it affects figure grouping).
  • Created atoms/ocr-canonical-roles.md with full canonical role list.
  • Updated workflows/ocr-truth-audit.md: pre-flight checklist now requires knowing canonical roles; truth_role must be from the canonical set.

Problem: Gates 1-4 were verified on known corpus but needed validation against unseen papers to prove generalization rather than overfitting.

Root cause: Standard pre-release validation gap — known-fixture green doesn't guarantee real-world generalization.

Fix: Selected 5 unseen papers from the vault (not in existing audit corpus), covering different domains and layout complexities:

For each paper: rebuilt pipeline, generated annotated pages, performed visual block review via vision agent, wrote block_review.jsonl with truth assessment, and verified coverage (all PASS at ratio=1.0).

Result: Blind audit passes. No new failure families discovered. All body text correctly classified, all reference zones ACCEPT, all body anchors ACCEPT. Issues found are role-label granularity (sidebar tags, table legends page), not zone-level or content-loss errors.

Test fixtures added: block_trace.csv for all 5 blind audit papers.

Next topic: OCR-v2 can be declared "state healthy" on known + unseen layout classes. Candidate for merge to main after final lint/type pass.

10.15 Remaining Known Issues (Post Blind Audit)

  • ~40 blocks across corpus where audit truth is stale (pipeline correct, truth wrong — e.g., noise→body_paragraph where text is clearly body content)
  • ~50 blocks across corpus that are genuine edge cases: backmatter boundary, caption candidate promotion, non-body insert classification — all low-severity
  • PZ8B59K4 has 34 sidebar blocks with no zone (publisher table-number sidebar); figure_unknown_000 render for unmatched figure

  • All issues are known patterns — no new failure families from blind audit

11.0 OCR Maintenance UX Redesign (2026-06-19)

Problem: The plugin OCR maintenance tab exposed raw OCR state fields (green/yellow/red, done_degraded, derived_stale) and pushed ordinary users toward interpreting any warning-like signal as a repair task. The UX was organized around backend table rows instead of user-facing actionability.

Root cause: The tab used badge-based status classification as its primary organizing axis, mixing actionable failures with non-actionable quality caveats behind similar visual warnings.

Fix: Added a UI-side categorization layer (paperforge/plugin/src/services/ocr-maintenance-ui.ts) that maps backend rows into four user-facing categories: No Action Needed, Rebuild Recommended, OCR Failed, and Result Limited. Replaced table-first rendering with summary-first layout: hero conclusion card, Needs Attention section (rebuild/failed only), Result Limitations section, and collapsible advanced table. Added new i18n keys in both en/zh. CSS styles for cards, chips, and counts section.

UX rules implemented:

  • recommended_action === 'rebuild' → promoted as proactive maintenance
  • recommended_action === 'redo' only surfaced for true failure states
  • degraded/warning signals without clear action → Result Limited, no maintenance button
  • Redo is not aggressively promoted for non-failed papers

Files changed: ocr-maintenance-ui.ts (new), ocr-maintenance-ui.test.ts (new), settings.ts (+289/-186), i18n.ts (+26), styles.css (+101), main.js (rebuilt)

Problem: 452 rebuild papers showed 5% green / 61% yellow / 34% red health scores. Needed comprehensive analysis of all failure modes before attempting fixes.

Audit scope: 18 dimensions across all 452 rebuild papers:

  • Health color distribution and issue combinations
  • Figure matching (76% match rate, 6 root causes)
  • Table matching (71% match rate, truncated caption issue)
  • Orphan explosion (6021 orphans, 93% papers affected)
  • Heading counting bug (only counts section_heading, ignores subsections)

  • ocr_render.py:604: column sort swaps adjacent refs via x_left tiebreaker
  • ocr_tables.py:16: _TRUNCATED_TABLE_ONLY_PATTERN rejects "Table N" format

Recommended fix priority (two batches):

  • Batch 1 (P0/P1): footnote removal + table note consumption, table caption no blockquote, heading count fix, Table N geometry matching, supplementary namespace split, page_assets gate
  • Batch 2 (P2): reference numbering + sort, figure/table asset arbitration, health ratio/weighted, completeness 7-bucket outcome, references_found zone check

11.2 Rebuild Audit Remediation Batches 1-5 (2026-06-19)

What was done: Executed the 5-task remediation plan for rebuild-output hardening.

  • Task 1: Carried owned table notes through inventory → object markdown → fulltext skip
  • Task 2: Removed display-zone table-caption blockquote duplication; fixed heading count and reference evidence in health

11.4 Table Note Stabilization + Table Ambiguity Slice (2026-06-20)

Problem: Table notes still risked confusion with page footnotes and body text, while bare Table N captions still stayed overly ambiguous after the first rebuild-hardening pass.

Root cause: The table surface lacked a page-footnote prior, grouped note-band ownership, and stronger layout tie-breaks among already-accepted table candidates.

Fix: Added page-footnote priors, table-below note-band grouping, body exclusion, explicit note-band contract fields, and stronger same-page / continuation tie-breaks for bare Table N.

Result: Table-note ownership is more stable, page-bottom footer notes are less likely to be absorbed, and table ambiguity is reduced through geometry rather than freer caption admission.

Validation: Rebuilt 7 residual and unseen papers after the change; no new failure family introduced.

11.5 Region-Growing Figure Merge Slice (2026-06-20)

Problem: Row-first pair/triple grouping had become safe after page_assets gating, but still under-modeled irregular multi-panel figures and left figure ownership recall on the table.

Root cause: Candidate groups were still built around neat-layout assumptions and lacked a seed-growth model plus post-growth validation.

Fix: Added seed-based local region growth, retained merge evidence per absorbed asset, validated grown groups before strict ownership, and demoted or split suspicious merges rather than forcing them.

Result: Figure grouping is less dependent on tidy layouts while preserving the no-page-swallow guardrail.

Validation: Rebuilt residual and unseen figure-heavy papers after the change; no new failure family introduced.

11.6 Reference Zone And Ownership Hardening With Untouched-Paper Regressions (2026-06-20)

Problem: Untouched truth-audit papers still showed reference intrusion and ownership fragmentation on irregular layouts. Root cause: Page continuity facts were too weakly shared across reference and ownership consumers. Fix: Added shared layout facts, hardened practical reference corridor containment, introduced journal-abbreviation-backed reference-style support, added bridge-aware display continuity for figure/table ownership, and limited PDF text fallback to local reference-entry repair. Result: Reference handling is more containment-oriented on mixed pages; ownership tolerates bridgeable empty gaps without reopening page swallow. Test status: Targeted OCR unit tests and untouched-paper regressions pass.

11.7 Figure Matching Hardening: Inline-Mention, Table-Label, Sub-Panel, Cross-Page, Legend-Bundle (2026-06-20)

Problem: 10-paper audit showed figure matching at ~75%. Dominant failure modes: (1) _looks_like_inline_figure_mention used text-content heuristics (verb lists, word-count thresholds) that falsely flagged long captions as body prose — caption_score dropped to 0.2, figures routed to unmatched_legends. (2) OCR labeled tabular sub-panel grids as tablemedia_asset role → excluded from figure-matching candidate pool. (3) Sub-panels fragmented into separate blocks across pages, never merged into parent figure clusters. (4) Cross-page figures (caption on page N+1, assets on N) unmatched by same-page-only matcher. (5) Preproof legend bundling (all captions on one page, figures on later pages) undetected.

Root cause (inline mention): _looks_like_inline_figure_mention line 91 used len(words) >= 10 as gate for body-prose detection. Long VFS figure captions (30+ words) hit this, caption_score dropped below 0.4 threshold.

Root cause (table label): build_figure_inventory excluded media_asset with raw_label="table" from both assets collection (line 1000) and _media_clusters (line 432). KZP6FB4Y Fig 2 is a tabular sub-panel grid labeled table by PaddleOCR.

Root cause (sub-panel merge): _grow_region_from_seed had gap threshold of page_width * 0.03 = 36px. Sub-panels ~50px apart in multi-panel figures couldn't merge.

Root cause (cross-page): _allow_previous_page_sequential_match required zone == "post_reference_backmatter_zone". 28ALPCY7 Fig 6 caption in display_zone blocked. Also future_page_asset had no page-distance guard.

Root cause (legend bundle): PZ8B59K4 has "Figure Legends" page (p16) with 5 captions, figures on p19-23. Dedup logic preferred body-mention over figure_caption role, losing Fig 3. Sequential fallback matched across 3-page gaps.

Fixes:

  • _looks_like_inline_figure_mention: replaced text-content heuristics with zone+style signals. Functions with figure_caption role or display_zone + legend_like style return False directly.
  • ocr_figures.py: added "table" to media_asset raw_label filter in 3 places (assets collection, _build_candidate_figure_groups_from_assets, _media_clusters).
  • _grow_region_from_seed: increased gap threshold from 36px to max(page_width * 0.08, 40) = 96px.
  • close_asset_tie and is_legend_only both filter figure_caption/figure_caption_candidate with fig_num is None to unmatched_legends.
  • _allow_previous_page_sequential_match: added display_zone to accepted zones.
  • future_page_asset: restricted to cp+1 only (no multi-page gaps).
  • Legend-bundle detection: 3+ figure captions on same page with 0 assets → 1:1 page-order match to subsequent pure-figure pages.
  • Dedup: prefer figure_caption role when both entries have no same-page assets.
  • marker_signature fallback: when _extract_figure_number returns None from empty text, check marker_signature.type == "figure_number".
  • ocr_pdf_spans.py: set role = "body_paragraph" on backfilled blocks. _refresh_artifacts runs backfill before rebuild_from_raw.

Test status: 341 passed (97 figure + 205 document + 69 roles + 11 PDF + 34 tables). 2 tests updated to match new inline-mention behavior (text-only blocks without zone/style now match instead of being unmatched).

Result: Figure matching 76/83 = 92% across 10 papers. VFS8CBW2: 3→8 matched. Remaining: RKSLQRIM 5 ambiguous (pre-proof cross-page), 6DIINFHX 1 ambiguous (close_asset_tie).

11.8 OCR Body Text Recovery: PDF Backfill For OCR-Missed Blocks (2026-06-20)

Problem: PaddleOCR detects layout regions but fails to extract text on some two-column pages. Span metadata exists (88 spans) but all have text="". Blocks end up as unknown_structural with text=[].

Root cause: Upstream OCR model limitation — column boundary regions have detectable bboxes but the OCR engine produces no text output.

Fix: ocr_pdf_spans.py backfills text from PDF embedded text layer using fitz.get_text("words", clip=bbox). Sets _ocr_raw_status = "missing_text_recovered" and role = "body_paragraph" on recovered blocks. _refresh_artifacts in audit runs backfill BEFORE rebuild_from_raw so structured block builder sees recovered text.

Result: KIX7SKXQ p3:7 recovered 323 chars ("Studying and understanding EnEF..."), p1:17 recovered 373 chars. All 4 papers audited show 0 empty unknown_structural blocks.

11.9 Figure Matching Known Issue: Shared-Caption Multi-Panel Figures (2026-06-20)

Problem: Paper 6QNRHRKX (JBJS 1970) has a 2x2 X-ray grid with individual "Fig. N" sub-panel labels (N=4,5,6,7) and a single shared caption below the entire grid. Each "Fig. N" label is treated as a separate truncated legend, creating 4 ambiguous figures with 4-7 candidates each. The real caption (block p3:12, figure_caption_candidate) can't match all 4 images simultaneously.

Scope: Rare pre-1990 format. Only observed in 6QNRHRKX across 36 audited papers.

Current mitigation: Truncated legends filtered when 2+ on same page. Short (<80 char) captions with fig_num=None go to unmatched_legends. Long shared caption remains as is.

Targeted fix (deferred): Detect N truncated legends sharing one long caption on same page → merge all N images as sub-panels of one composite figure with the long caption as owner. Trigger: >=2 `_TRUNCATED_LEGEND_ONLY_PATTERN` entries on same page + >=1 figure_caption_candidate with text >80 chars spanning the full grid width.

11.10 Figure Matching Known Issue: Cross-Page Preproof (RKSLQRIM) (2026-06-20)

Problem: RKSLQRIM (preproof) has 5 remaining ambiguous figures. Fig 10 caption on p28 has 0 same-page candidates. Sub-figure panel labels (A, B, C) on p15, p19, p20, p24 are misclassified as figure_caption with empty text. These block types: display_zone + legend_like + raw_label=figure_title — the pipeline sees a legend with empty text and a marker_signature with the figure number, but the figure assets are on different pages.

Scope: Preproof papers with cross-page figure layouts. Affects ~5-7 papers across 36.

Current mitigation: marker_signature fallback extracts figure numbers from empty-text blocks. These now get single_unconfirmed_match with 1 candidate instead of no_asset_match. But cross-page matching still fails for pages separated by body text.

Targeted fix (deferred): Extend sequential fallback or legend-bundle to handle cross-page gaps larger than 1 page when intervening pages have no competing figure captions.

11.11 Figure Matching: close_asset_tie Above-Preference + adjacent_x Scoring (2026-06-20)

Problem 1 (close_asset_tie): When a figure caption is sandwiched between two asset groups (one above, one below), both scored identically and the algorithm couldn't break the tie. This produced close_asset_tie ambiguous entries for standard layouts where the caption sits below the figure image and above the next figure's image.

Fix: When close_asset_tie fires with both "above" and "below" sides, prefer the above-only candidates (standard: image → caption stacked below). Falls back to close_asset_tie only when no above-only candidates exist.

Result: 2AGGSMVQ Fig2+Fig5 fixed (4/6→6/6), 2H8MZ27H Fig1 fixed (5/6→6/6). 97 figure tests pass.

Problem 2 (side-by-side caption): Two-column journals (JSES International, Springer CORR) place figure captions in the left column with the image in the right column at the same y-band. score_figure_match had zero x_overlap for this layout because bounding boxes don't intersect horizontally. The decision gate at score >= 0.6 AND (has_x_overlap OR ...) blocked the match because has_x_overlap=False and contextual_support=False.

Fix: Added adjacent_x signal to ocr_scores.py:score_figure_match: when horizontal gap < 80px AND < 30% of narrower bbox width, treat as horizontal adjacency → set has_x_overlap=True and add +0.20 score. This is a side-by-side layout detection, not a column segmentation, so it doesn't require stable page-level column boundaries.

Result: 4DU8LEH2 Fig1 (JSES, caption-left image-right) fixed: 3/4→4/4. 4KCHGV2Z Fig2 (Springer CORR, same pattern) fixed. 302 tests pass.

11.12 Figure Matching Known Issue: Multi-Column Figure Over-Merge (2026-06-20)

Problem: 3FDT9652 (JSES International, 2-column journal). Page 3 has Fig 2 (left column) + Fig 3 (right column) at the same y-band. The page_assets / _grow_region_from_seed logic merges both images into one cluster because they share y-ranges, even though they belong to different figures in different columns. This causes Fig 2 to steal Fig 3's image, Fig 3 to steal Fig 4's images below, and Fig 4 to go ambiguous (0 assets left). All 6 figures on pages 2-5 are affected.

Root cause: Matching uses vertical proximity only, without column-boundary awareness. On multi-column pages, figures in different columns at the same y get incorrectly merged. The _grow_region_from_seed function's adjacent_right check (gap ≤ 96px) happily merges across column gutters.

Why not fix with column segmentation: Column gutter positions vary by journal (JSES ~750px, Springer ~650px, 3-column layouts ~450px/750px). A hardcoded threshold regresses other papers. Multi-column layouts are also relatively rare in the current collection (1 of 37 normal papers).

Scope: Affects 3FDT9652 specifically. Not observed in the other 36 normal papers.

Targeted fix (deferred): Requires "caption-as-boundary" matching: when N captions exist on a page with N asset clusters, match by reading-order proximity rather than all-page-assets clustering. Each caption should act as a firewall that prevents assets from being claimed by captions above it. Alternatively, detect multi-caption pages and disable page_assets groups (which merges all page assets into one cluster) for those pages, relying on same_row_* + region-grown groups only.


9.3 Figure Merge Root-Cause Analysis (2026-06-21)

The problem

Multi-panel figures on complex layouts (SAN9AYVR, 2GN9LMCW Fig 4) fail to merge all sub-panels. Unmatched assets remain. The 4-direction growth + scoring bonus fixes didn't solve the core issue.

Why human vision always merges them

A human looking at a page sees:

  1. A cluster of images close together → ONE perceptual group
  2. ONE legend/caption for that cluster → confirms the group
  3. Gaps between images in the same figure < gaps to the next figure/text
  4. The cluster forms a rectangular bounding box

Result: the human always merges, no competition.

Why the algorithm fails

The grouping has 3 competing layers:

Root cause: The algorithm treats grouping as a LOCAL greedy problem when it should be a GLOBAL clustering problem.

A human sees ALL assets, computes global distances, and forms clusters. The algorithm:

  1. Generates overlapping groups (same asset can be in single_asset, same_row, AND region_grown)
  2. Makes them COMPETE via scoring (single_asset vs merged group for the same legend)
  3. The scoring favors closeness (single asset close to caption wins over merged group with larger centroid distance)

Fix direction: Replace greedy competition with global spatial clustering.

Edge-case analysis: risks and fallbacks

The simple clustering approach (group all assets within a distance threshold) fails on multiple real layouts. Here's a complete risk matrix:

12.1 Figure Merge: Greedy Region-Growth → Global Distance Clustering

Problem: _build_candidate_figure_groups_from_assets used greedy seed-growth (first seed wins, eats adjacent regardless of figure boundary). Failed on multi-panel figures with irregular layouts.

Fix: Replaced with global distance clustering via union-find:

  • Added helpers: _rect_intersection_area, _has_text_separator, _filter_figure_assets, _cluster_page_assets
  • Clusters by distance (horizontal < 12% page_width, vertical < 8% page_height), text-separator-aware
  • Added scoring branch for distance clusters (auto-match at 0.98, skip legacy)
  • Group-aware sequential fallback: searches any future page, not just cp+1

  1. P1:8 eaten — Block 8 on page 1 (right column body paragraph continuation, seed_role=body_paragraph) was misclassified as non_body_insert. Root cause: _detect_non_body_insert_clusters first-pass page-1 guard uses "-" in text (any hyphen anywhere, e.g. "bone-related"), not startswith. Fix in ocr_document.py: change any(m in text.lower() for m in ["\u2022", "-", "*"]) to text.startswith(("\u2022", "- ", "* ")) AND add position+style check (font matches body spine + width > 20% page) instead of naive length check.

  2. P11:5,6 MOF paragraph duplication — Block 5 (left column, OCR text=full 1637-char paragraph) + Block 6 (right column, text_source="pdf_text_layer_fallback" with 1010-char fragment) both render the same overlapping content. Root cause: backfill_missing_text_from_pdf uses get_text("words", clip=expanded) which returns ANY word with bbox overlapping the clip rect, even if the word text belongs to a larger paragraph that extends beyond the block's bbox. Fix needed: filter backfilled words to only those strictly within the block's original bbox, OR handle dedup at render level.

  3. Callout font threshold too tight0.9 * body_font_median missed footnote bid=9 (fn_fs=7.97 < 8.84*0.9=7.956 by 0.014). RESOLVED: changed to fn_fs < body_font_median.

12.15 OCR rebuild low-risk de-dup pass (2026-06-22)

Problem: Derived rebuild spent avoidable time reopening PDFs during object extraction, writing structured blocks twice, reading frontmatter candidates back from a just-written JSONL file, and rescanning structured blocks during heading-prefix override in render.

Root cause: Expensive resources and intermediate artifacts were owned below the correct batch boundary, so rebuild paid repeated PDF, render, write, and lookup costs even when the final outputs did not require them.

Fix: Reused one lazily-opened shared PDF handle per extract_and_write_objects() call, preserved cache-first crop semantics, switched rebuild metadata-candidate extraction to an in-memory helper, removed the intermediate structured JSONL write, and replaced the render heading-prefix rescan with direct bid_to_block lookup.

Result: Rebuild preserves the same output semantics while removing repeated PDF opens, repeated same-page setup work, one intermediate structured write/read cycle, and one safe O(n^2) render lookup.

Test status: Added focused regression coverage for cache-first no-open behavior, open-once reuse, same-page render reuse, markdown survival on PDF-open failure, in-memory/file-based frontmatter-candidate parity, single structured-block write in rebuild, and heading-prefix parity in render.

12.16 Derived rebuild span-backfill skip contract (2026-06-22)

Problem: Derived rebuild reran PDF span backfill on every rebuild even when raw blocks already contained valid span enrichment.

Root cause: Rebuild had no explicit validity contract for stored span enrichment, so it treated all rebuilds as requiring a fresh PDF scan.

Fix: Added explicit skip/rerun policy using span-backfill version, visual-container version, current PDF fingerprint from compute_pdf_fingerprint(), eligible text-like coverage, recorded span_backfill_status fields in meta.json, and protected the meta write order so validity fields are only refreshed after raw persistence succeeds.

Result: Rebuild can reuse valid stored raw enrichment, avoid unnecessary PDF scans, and record why skip or rerun happened.

Test status: Added focused rebuild tests for eligible coverage denominator, valid skip, version/fingerprint/coverage/visual-container mismatches, unknown fingerprint invalidation, PDF-missing unavailable status, and raw-write failure validity protection.

12.17 Caption-independent semantic figure grouping Stage 1A (2026-06-22)

Problem: build_figure_inventory() still fed candidate_groups from caption-band-local partitions on multi-caption pages, so ledger/reservation truth was already caption-conditioned before matching.

Root cause: _build_candidate_figure_groups_from_assets() mixed semantic grouping truth with _partition_assets_by_caption_bands(...) and caption-count-aware topology, letting local caption bands masquerade as ownership groups.

Fix: Added caption-independent semantic grouping helpers in paperforge/worker/ocr_figures.py, introduced topology-by-asset_block_ids test seam, kept caption-band data as metadata-only assist keyed by semantic groups, and rewired _build_candidate_figure_groups_from_assets() so candidate_groups now comes from semantic groups rather than band-local partitions.

Result: Same visual assets now keep stable semantic topology across caption-count changes; neutral text barriers still split continuity; ordinary same-page two-figure layouts remain separate; caption-band-local groups no longer enter the main ownership input stream.

12.18 Stage 1B grouped-asset fallback guard (2026-06-22)

Problem: Stage 1B reservation and cross-page settlement were already operating on semantic groups, but the preproof legend_bundle fallback still consumed raw unmatched_assets without checking whether those assets already belonged to semantic groups.

Root cause: The late old sequential fallback had a grouped-asset exclusion, but the earlier legend-bundle fallback rebuilt its own asset-page list directly from unmatched_assets, so grouped assets could still be stolen before the group-aware fallback ran.

Fix: Added _grouped_asset_page_ids(candidate_groups) in paperforge/worker/ocr_figures.py, reused it for the old sequential fallback, and applied the same grouped-asset exclusion to the legend-bundle fallback. Added a narrow regression in tests/test_ocr_figures.py proving legend-bundle cannot consume assets from a semantic group.

Result: Stage 1B now keeps semantic-group ownership intact across the remaining late fallback paths: reserve/same-page/cross-page continue to use semantic groups, and legacy fallback paths no longer steal grouped assets.

12.15 Stage A Ownership Registry Mirror (2026-06-22)

  • Problem: figure ownership state in ocr_figures.py was mutated through scattered used_group_ids and used_asset_page_ids writes, so Stage B/C governance work had no explicit ownership seam to build on.
  • Root cause: ownership lived only as ad hoc set updates inside same-page, cross-page, sidecar, bundle, and sequential figure matching paths.
  • Fix: added FigureOwnershipRegistry in paperforge/worker/ocr_figures.py and routed Stage A figure-ownership commits through it while keeping the existing used_group_ids / used_asset_page_ids sets as the mirrored truth surface for current green flows. Added tests in tests/test_ocr_figures.py for blocked-asset reasons, conflicting figure/table ownership rejection, and registry mirror agreement with the current used-set contract.
  • Result: Stage A now has an explicit ownership helper API without changing matched-figure behavior on the verified green ownership/grouped cases.

12.16 Stage B Local Pairing Hypothesis Surface (2026-06-22)

  • Problem: same-page figure matching only existed as immediate commit-time geometry/scoring decisions, so Stage C had no explicit local hypothesis surface to validate or defer.
  • Root cause: caption_below, caption_above, and sidecar interpretation were implicit in scattered matching branches and never exposed as side-effect-free objects.
  • Fix: added _make_local_pairing_hypothesis() and _infer_local_pairing_mode() in paperforge/worker/ocr_figures.py, collected non-authoritative local_pairing_hypotheses during same-page candidate evaluation, and surfaced sidecar partition hypotheses without changing current ownership commit flow. Added focused tests in tests/test_ocr_figures.py for pure below/above/sidecar hypothesis construction plus a mixed-page fixture proving sidecar and below hypotheses can coexist on one page.
  • Result: Stage B now exposes explicit local pairing hypothesis objects while keeping ownership mutation on the existing paths.

12.17 Stage C Reservation-Aware Same-Page Commit Gate (2026-06-22)

  • Problem: reservation outranked same-page commit only by skipping reserved legends/groups before local evaluation, so reserved same-page interpretations never surfaced as explicit deferred hypotheses and one reserved no-asset path could leak into ambiguous state before cross-page settlement.
  • Root cause: same-page scoring and same-page commit were still fused; reserved objects were excluded too early instead of being evaluated then withheld at commit time.
  • Fix: added _mark_hypothesis_conflict() in paperforge/worker/ocr_figures.py, allowed reserved same-page candidates to be scored into local_pairing_hypotheses, tagged them with reserved_same_page_commit_deferred, and inserted a narrow defer-at-commit gate so reserved legends/groups stay in the cross-page settlement flow instead of committing or falling into early ambiguous/unmatched buckets.
  • Result: reservation now outranks same-page commit through an explicit gate, while non-reserved same-page matches still commit normally and 2HEUD5P9 keeps the intended cross-page ownership behavior.

12.18 Stage D1 Shared Fallback Eligibility Helpers (2026-06-22)

  • Problem: late fallback paths each performed their own ownership checks, so grouped/pre-owned/blocked objects were filtered inconsistently.
  • Root cause: there was no shared helper layer for fallback eligibility; each path mixed ownership logic into local control flow.
  • Fix: added _fallback_eligible_asset_page_ids(), _fallback_can_consume(), and _fallback_eligible_groups() in paperforge/worker/ocr_figures.py as the shared ownership boundary layer for later fallback stages.
  • Result: fallback stages now have one consistent helper surface for rejecting pre-owned, blocked, and grouped objects.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "fallback and eligible" -v --tb=short -> 3 passed.

12.19 Stage D2 Sidecar Fallback Guard (2026-06-22)

  • Problem: sidecar fallback could reclaim too much page media and was willing to reinterpret a page based on narrow captions without enough local row-coupled evidence.
  • Root cause: sidecar partitioning operated on broad page bands and had no explicit reclaim/eligibility layer when replacing earlier narrow-caption ownership.
  • Fix: added _caption_row_coupled_assets() and rewired the sidecar fallback to use D1 eligibility helpers plus narrow reclaim of only the ownership it is explicitly replacing.
  • Result: sidecar remains available for true narrow sidecar layouts while mixed local layouts can keep ordinary below pairs separate.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "6FGDBFQN or sidecar or 3FDT9652" -v --tb=short -> 5 passed; python -m pytest tests/test_ocr_real_paper_regressions.py -k "6FGDBFQN or 3FDT9652" -v --tb=short -> 2 skipped in this worktree because fixture prerequisites are absent.

12.20 Stage D3 legend_bundle Fallback Guard (2026-06-22)

  • Problem: bundle fallback needed explicit confirmation that grouped assets, interrupted pages, and pre-owned asset pages remain unavailable.
  • Root cause: the bundle path relied on local inline checks but had no dedicated regression coverage for those ownership and interruption boundaries.
  • Fix: added focused bundle regressions covering grouped-asset exclusion, pre-owned asset-page exclusion, and body interruption rejection.
  • Result: the existing narrowed bundle path remains green under the new ownership/interruption contract.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "legend_bundle" -v --tb=short -> 3 passed.

12.21 Stage D4 Sequential Guard (2026-06-22)

  • Problem: sequential fallbacks needed explicit proof they do not cross ownership boundaries once earlier paths have already claimed an object.
  • Root cause: earlier tests covered grouped-asset protection but not the pre-owned bare-asset case explicitly.
  • Fix: added test_sequential_fallback_skips_preowned_bare_asset() to lock the old sequential path behind the same ownership boundary.
  • Result: both group-aware sequential and old sequential now have focused regression evidence for ownership-safe behavior.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "group_sequential or sequential" -v --tb=short -> 4 passed.

12.22 Stage D5 sequence_match Contract (2026-06-22)

  • Problem: sequence_match promotion could create a matched figure shell without carrying the real ownership payload required by downstream consumers.
  • Root cause: _promote_sequence_matches() only checked adjacent numbering plus asset_block_ids, then emitted a placeholder matched figure with empty matched_assets and no legend_page / asset_pages / settlement_type contract.
  • Fix: tightened _promote_sequence_matches() so promotion now requires concrete matched_assets, asset_pages, legend_page, and page, and emits a full contract with asset_block_ids and settlement_type="sequence_match"; otherwise the item stays ambiguous.
  • Result: sequence_match is now either a real matched-figure payload or not promoted at all.
  • Tests: python -m pytest tests/test_ocr_figures.py tests/test_ocr_figure_reader.py -k "sequence_match" -v --tb=short -> 3 passed.

12.23 Stage E Figure/Table Ownership Conflict Surface (2026-06-22)

  • Problem: figure/table overlap had no explicit post-hoc conflict surface, so dual ownership would stay silent.
  • Root cause: figure and table inventories were produced independently and never compared after matching.
  • Fix: added _collect_figure_owned_asset_ids(), _collect_table_owned_asset_ids(), _build_ownership_conflicts(), and attach_ownership_conflicts() in paperforge/worker/ocr_figures.py, then wired the conflict attachment into paperforge/worker/ocr.py and paperforge/worker/ocr_rebuild.py after table inventory construction.
  • Result: one-owner violations are now surfaced explicitly as ownership_conflicts instead of remaining invisible.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "ownership_conflict or table" -v --tb=short -> 1 passed.

12.24 Stage P0-A Conflict Persistence Order (2026-06-23)

  • Problem: no test existed to confirm ownership_conflicts was persisted in the written figure_inventory.json artifact; the in-memory test alone could not catch a write-order regression.
  • Root cause: write-order was already correct (attach before write) in both ocr.py and ocr_rebuild.py, but the absence of a persisted-artifact test meant future reordering could silently drop the field.
  • Fix: added test_ownership_conflicts_persisted_in_figure_inventory_json that exercises attach_ownership_conflicts + write_figure_inventory + read_json, asserting the written file carries ownership_conflicts with the expected list shape and content.
  • Result: persisted-artifact contract is now explicitly enforced. No code changes needed in production path.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "ownership_conflict" -v --tb=short -> 2 passed.

12.25 Stage P0-B Bundle-Source Duplicate Legend Canonicalization (2026-06-23)

  • Problem: bundle-source legends (caption pages with 3+ figure legends and zero assets) that had NO stronger duplicate elsewhere were excluded from the legend_bundle fallback. After dedup removed sibling bundle captions, only 1-2 legends remained on the page, falling below the hard len(caps) >= 3 threshold. Additionally, the fallback excluded all assets in candidate groups (grouped_asset_page_ids), including single-asset orphans on subsequent pages.
  • Root cause: (1) the bundle fallback threshold len(caps) < 3 did not account for deduped bundle-source pages where survivors drop below 3. (2) the grouped_asset_page_ids filter excluded all candidate-group assets, even single_asset groups on pages without same-page captions which are functionally unclaimed.
  • Fix: (1) lowered the threshold for bundle-source pages (pages with any bundle_source_legend_ids captions) to allow 1+ legends to trigger the fallback. (2) replaced grouped_asset_page_ids with cluster_asset_page_ids (only distance_cluster groups) so single-asset orphans on subsequent pages remain available for bundle consumption while true multi-asset clusters stay protected.
  • Result: bundle-only source legends now reach the legend_bundle fallback; dedup losers still properly defer to stronger real duplicates; multi-asset clusters remain unbroken; DWQQK2YB figure 3 ownership unaffected.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "bundle_source_duplicate or bundle_only_source or deduped_duplicate or same_page_real_legend" -v --tb=short -> 3 passed. python -m pytest tests/test_ocr_figures.py tests/test_ocr_figure_reader.py tests/test_ocr_render.py -q -> 174 passed. DWQQK2YB regression passes.

12.26 Stage P1A Composite Parent Detector (Diagnostic-Only) (2026-06-23)

  • Problem: composite (multi-panel) figures were not modeled beyond atomic semantic grouping. Pages with columnar stacks, grids, or vertically aligned sub-panels had no parent-candidate surface, leaving downstream ownership paths blind to composite structure.
  • Root cause: the visual grammar layer stopped at atomic distance-based clustering (distance_cluster / single_asset), with no higher-order same-page parent candidate construction.
  • Fix: added _build_composite_parent_figure_groups_visual_only() that clusters atomic groups on the same page using horizontal alignment (x-overlap >= 50%) plus height/width similarity. Connected components with ≥2 groups become composite_parent candidates with confidence scores (0.50-0.75, capped). Strictly visual-only — no caption/legend identity used in topology. Wired into build_figure_inventory as composite_parent_candidates output field. All candidates have ownership_enabled: False (diagnostic-only, P1B enables ownership).
  • Result: audit-visible composite parent surface available without changing any ownership behavior. 174 baseline tests unchanged. DWQQK2YB regression unaffected.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "composite_parent or mega_merge" -v --tb=short -> 4 passed (dense grid detection, stacked vertical panels, mega-merge prevention, ownership-count preservation). Real-paper regression tests for VFS8CBW2/24YKLTHQ/2UIPV93M not yet in suite — synthetic fixtures cover the representative layouts.
  • Fixture absence note: no real-paper regression tests existed for VFS8CBW2, 24YKLTHQ, or 2UIPV93M. The 4 synthetic tests in test_ocr_figures.py provide coverage for dense composite grids, vertically stacked panels, ordinary multi-figure page separation, and diagnostic non-interference.

12.27 Stage P1B Composite Parent Ownership Arbitration (2026-06-23)

  • Problem: P1A diagnostic-only composite parents had no ownership path. Strong parent candidates (evenly spaced, same-width sub-panels) could not claim their children, leaving assets matched individually or orphaned.
  • Root cause: composite parent candidates were only diagnostic output. No arbitration path existed to promote strong parents into ownership.
  • Fix: inserted parent arbitration before same-page candidate scanning in the per-legend matching loop. For each legend with a figure number, check if a composite_parent_candidate exists on the same page. If parent confidence >= 0.65, page has no competing captions (>1 numbered legend), and parent is unclaimed: accept the parent, consume all child groups/assets, emit a settlement_type="composite_parent" match, and skip normal candidate scanning. Weak parents (confidence < 0.65) or pages with competing captions fall through to normal atomic matching. Forbidden: composite_parent_candidates never flow into legacy fallback paths.
  • Result: strong composite parents now claim all child assets in one match. The forbidden approach (candidate_groups = atomic_groups + composite_parent_candidates) was avoided — parents are arbitrated in a separate pre-scan, not commingled with atomic groups. 182 baseline tests pass, 0 regressions.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "parent or veto" -v --tb=short -> 8 passed (acceptance, weak rejection, competing-caption veto, no-legacy-fallback).

12.28 Stage P2 Mixed Caption Grammar Validator (2026-06-23)

  • Problem: pages with mixed caption grammar styles (e.g. "Figure 1." vs "Fig 1.") had no annotation surface, so downstream could not detect inconsistent figure/table caption patterns.
  • Root cause: local pairing hypotheses carried no grammar consistency metadata.
  • Fix: added _validate_page_local_caption_grammar() that groups hypotheses by page, extracts caption prefix style (e.g. "figure" vs "fig"), and annotates each hypothesis with grammar_status (accepted/deferred/conflict), grammar_reason, and grammar_evidence. Uniform pages get "accepted", mixed pages get "conflict", pages with no detectable grammar get "deferred". Annotation-only — no matching behavior change.
  • Result: all local_pairing_hypotheses now carry grammar metadata for audit/debugging. 185 baseline tests pass.
  • Tests: python -m pytest tests/test_ocr_figures.py -k "grammar_status or mixed_page or grammar" -v --tb=short -> 4 passed (annotation contract, conflict detection, uniform-page consistency, sidecar preservation).

12.29 Stage P3A Asset Family Hint (2026-06-23)

  • Problem: figure/table separation had no signal about whether an asset is intrinsically figure-like or table-like based on its raw label.
  • Root cause: assets carried raw_label but no downstream-usable family hint.
  • Fix: annotated each figure asset after collection with asset_family_hint (figure_like/table_like/ambiguous), asset_family_confidence, and asset_family_evidence based on raw_label. Image/chart/figure → figure_like (0.70), table → table_like (0.70), no label → ambiguous (0.35). Hints are persisted in figure_inventory.json asset records.
  • Result: audit-visible family hints available for P3B/C veto and future figure/table arbitration.

12.30 Stage P3B/P3C Figure/Table Separator Veto (2026-06-23)

  • Problem: figure matcher could claim table-like assets, creating ownership conflicts with the table matcher.
  • Root cause: no pre-match veto existed to prevent figures from consuming table-like regions.
  • Fix: in the same-page candidate scanning loop, after ownership check and before scoring, compute whether ALL assets in a candidate group are strongly table-like (asset_family_hint == "table_like" AND confidence >= 0.70). If so, skip the group (veto). Ambiguous assets pass through. This is a down-rank approach — only strong table-like signals are hard-vetoed.
  • Result: table-like assets are now excluded from figure matching. 188 baseline tests pass (188/188).
  • Tests: python -m pytest tests/test_ocr_figures.py -k "asset_family_hint or table_like or ambiguous_region" -v --tb=short -> 3 passed (hint contract, strong table-like veto, ambiguous pass-through).

12.31 Live-paper residual triage reset (2026-06-23)

  • Problem: after the visual-grammar hardening pass, the remaining ugly papers were being treated too uniformly even though they represent very different kinds of residual risk.
  • Root cause: the project had not yet separated “worth optimizing for” papers from “good only as guardrail samples”, so the execution queue still risked overfitting to pathological wraparound layouts.
  • Fix: reclassified RKSLQRIM as a defensive guardrail paper rather than a perfection target, and elevated VFS8CBW2 as the highest-value residual vulnerability family because it exposes reusable weaknesses: panel text falsely promoted into caption candidates, dense multi-panel pages exploding into unresolved clusters, and formal-caption-plus-panel-title competition. Recorded the same triage boundary in project/current/ocr-v2-generalization-boundary.md.
  • Result: current closeout strategy is now explicit: VFS8CBW2 first, then 6FGDBFQN, then smaller tails like 3FDT9652 / 24YKLTHQ, while RKSLQRIM remains a regression/guardrail sample and DWQQK2YB / 2UIPV93M leave the urgent queue.
  • Tests: live artifact review only; no code-path assertions added in this bookkeeping step.

12.32 Dense composite parent candidate hardening (2026-06-23)

  • Problem: VFS8CBW2-class dense fragmented pages produce zero composite_parent_candidates because _build_composite_parent_figure_groups_visual_only only uses atomic candidate_groups as input and cannot see unresolved_clusters. This means dense composite figures are invisible to arbitration before it even starts.
  • Root cause: parent candidate construction was scoped to ordinary atomic groups only. Dense pages with many visual fragments that partially match produce unresolved_clusters — but these were invisible to parent construction.
  • Fix: added _build_dense_composite_parent_candidates(...) — a visual-only construction function that runs after unresolved clusters are available. It triggers on pages with a numbered legend and >= 4 visual fragments (atomic group assets + unresolved cluster media blocks), merges all visual envelopes into a compact dense composite parent candidate, and appends it to composite_parent_candidates with parent_subtype = "dense_composite" and ownership_enabled = False. _compute_grid_score(...) provides row/column regularity scoring.
  • What this does NOT touch: ordinary same-page arbitration, sidecar, persisted buckets (matched_figures / ambiguous_figures / unresolved_clusters), atomic grouping thresholds, panel-title suppression logic, settlement_type list. The existing _build_composite_parent_figure_groups_visual_only is unchanged.
  • Result: dense fragmented pages now produce audit-visible composite_parent_candidates with parent_subtype="dense_composite", unresolved_cluster_ids, fragment_count, compactness, grid_score, and construction_reason. Ordinary multi-figure pages do not gain page-wide dense parents (guard test verified). Full regression: 209 pass, 2 pre-existing fail (unchanged).
  • Tests: 5 new synthetic tests covering dense parent emission, ordinary page guard, unresolved cluster visibility, and contract field completeness. All existing composite_parent tests remain green.
  • Stop-condition check: no atomic grouping thresholds widened, no new settlement path, no sidecar changes, no bucket semantics changed, no literal label blacklist. All boundaries intact.

12.33 Figure ownership arbitration convergence (2026-06-23)

  • Problem: OCR-v2 figure ownership was converging on multiple ad-hoc settlement paths (dense composite, panel-title, sidecar, sequence-shell). Without a unified arbitration model, each new P1B/P2 fix risked adding another direct settlement behavior.
  • Root cause: no internal convergence layer existed — caption evidence normalization, candidate-source normalization, page-level arbitration metadata, and unified accounting semantics were each handled inline without a shared contract.
  • Fix: added convergence scaffolding per docs/superpowers/plans/2026-06-23-figure-ownership-arbitration-convergence-implementation.md, which adds convergence constraints rather than a new one-paper path. The scaffolding includes:
    • Task 1: _ownership_decision_metadata(...) — internal metadata adapter for attaching ownership-decision annotations to persisted entries without replacing bucket semantics.
    • Task 2: FigureOwnershipRegistry.soft_reserve_assets(...) / finalize_soft_reservation(...) / release_soft_reservation(...) — provisional soft reservation that blocks legacy fallback consumption during arbitration but may be superseded by a stronger candidate.
    • Task 3: _should_suppress_panel_title_candidate(...) — structural demotion of short unnumbered text inside visual envelopes from formal legend matching, retaining them as embedded figure text evidence. Emitted into suppressed_caption_candidates.
    • Task 4: dense parent candidate normalization — parent_subtype="dense_composite" under group_type="composite_parent", not a parallel matcher family.
    • Task 5: construction-time vs arbitration-time separation — _build_dense_composite_parent_candidates(...) (visual-only) and _score_dense_parent_candidate_against_local_ownership(...) (arbitration-time, uses coverage gain / leftover mass).
    • Task 6: assetless sequence shell demotion — shells with no asset payload stay in ambiguous_figures with hold_reason="assetless_sequence_shell", never increment official_figure_count.
    • Task 7: dense page arbitration regression — relaxed _dense_parent_single_group_ok threshold to fragment_count >= 4; dense parents now built before legend loop to participate in arbitration; dense parents excluded from pages with competing captions to prevent swallowing neighboring figures; unmatched assets on dense pages consolidated into unresolved_clusters with cluster_source="dense_page_leftovers".
  • Result: 216 tests pass (up from 211 at convergence baseline). No persisted buckets removed. No new settlement_type path created. No atomic grouping thresholds widened. No literal label blacklists.
  • Tests: 8 new synthetic tests covering ownership decision metadata, provisional soft reservations, panel-title suppression, construction/arbitration separation, dense parent collection, neighbor protection, and dense page leftover consolidation. All existing tests remain green.
  • Relationship to existing roadmap: this plan adds convergence constraints (not replacement behavior) to the visual-grammar hardening roadmap (docs/superpowers/specs/2026-06-23-ocr-visual-grammar-hardening-design.md). Task 4, Task 5, and Task 7 were gated on P1A composite-parent candidates already existing on the branch.

12.34 Phantom unresolved cluster dedup (2026-06-23)

  • Problem: unresolved_clusters built during the rejected_legendsunmatched_assets→cluster pass (line 3883) referenced blocks that were later matched to figures by same-page/cross-page/sequential settlement. The clusters were never cleaned up after matching completed, causing audit tools to report phantom "unresolved" clusters whose blocks were already owned by matched figures. VFS8CBW2 showed 9/9 phantom clusters; RKSLQRIM showed 1/1 phantom.
  • Root cause: cluster construction ran before final figure matching/ownership resolution. The _recompute_final_unmatched_assets pass removes matched blocks from unmatched_assets but does not filter unresolved_clusters itself.
  • Fix: added a dedup pass in build_figure_inventory (before inventory dict construction) that removes from unresolved_clusters any blocks already owned by figures in matched_figures. A cluster with zero remaining unowned blocks is dropped entirely.
  • What it does NOT touch: single-asset unmatched entries, ambiguous_figures, held_figures, composite parent logic, dense page consolidation. The unresolved_clusters bucket semantics are preserved — only already-owned blocks are filtered.
  • Result: VFS8CBW2: unresolved_clusters 9→0. RKSLQRIM: unresolved_clusters 1→0. 6FGDBFQN: single real unresolved cluster retained (no overlap with matched figures). 2UIPV93M: 0 unresolved clusters (unchanged).
  • Tests: test_unresolved_clusters_deduped_against_matched_figures — synthetic test verifying no block in unresolved_clusters also appears in matched_figures. Full figure stack: 217 passed, 0 failed.

12.35 Ticket closeout: remaining residual issues identified (2026-06-23)

Vision audit of VFS8CBW2/6FGDBFQN/2UIPV93M/RKSLQRIM identified two residual issues outside the convergence layer:

  1. 6FGDBFQN Fig 3 false negative (assetless_sequence_shell) — two visual assets exist (MR image + gross anatomy photo) on page 3, caption clearly visible. Pipeline fails to match because caption is in left column, assets are in right column above caption's y-band. Root cause: spatial matcher (same_page/sidecar local pairing) does not handle two-column layouts with assets right/above caption. → Separate ticket: "Mixed local caption grammar: opposite-column above/right asset pairing"
  2. 2UIPV93M page 49 unmatched X-ray assets — blocks 1 and 5 are media_asset with empty zone, captions sit directly below. Pipeline fails to match because role/zone classification does not route these into figure candidate context. → Separate ticket: "Figure asset role/zone normalization for ordinary caption-below X-ray pages"

These are NOT convergence-layer issues. The phantom cluster bug (fixed in 12.34) was the only convergence-layer diagnostic inaccuracy found by this audit pass.

12.37 Sidecar narrow column outlier tolerance + raw band (2026-06-23)

  • Problem: 6FGDBFQN page 3 (AJR three-column layout with narrow left captions and wide right assets) sidecar fallback never triggered. Two root causes: (1) A right-column body text block ("...Fig. 6...") misclassified as a figure caption broke _same_page_narrow_caption_column's x-center spread check, returning [] instead of the valid left-column cluster. (2) When sidecar did run, _caption_row_coupled_assets over-filtered far-column panels with x_gap > 0.25*page_width, dropping the rightmost asset in each figure's band.

  • Root cause: _same_page_narrow_caption_column used a one-shot spread check — any outlier x-center killed the entire narrow column. And sidecar promotion used row-coupled geometry designed for caption-below/above layouts, which is too strict for caption-column layouts.

  • Fixes: (1) Outlier-tolerant _same_page_narrow_caption_column: when the global x-center spread check fails, clusters formal captions by x-center and keeps the largest aligned cluster with >=2 captions. Singleton outliers (right-column body mentions) are dropped. (2) Sidecar promotion uses band_assets = raw_band_assets directly — the band partition is the ownership unit for sidecar pages, not row-coupled geometry. (3) Added asset_block_ids to sidecar entry schema (was missing, causing empty asset reporting).

  • Result: 6FGDBFQN Fig 3 now correctly matched with 2 assets [11, 13] via sidecar settlement. Fig 2 on same page correctly gets 2 assets [3, 4] via sidecar (previously 4 via same_page, incorrectly included Fig 3's assets). No same_page/cross_page/dedup changes. Row-coupled helper retained for other uses.

  • Tests: 4 new synthetic tests covering outlier-tolerant narrow column, 2-caption minimum, full raw band usage, and protected asset exclusion. 221 tests total, 0 failures.

  • Problem: the legend dedup (_dedup_map) uses (namespace, figure_number) as the unique key and unconditionally removes all but the highest-priority legend. When a paper has independent figures with the same number in different sections (e.g., body Figure 1 vs appendix Figure 1), the weaker style/zone variant is silently dropped. 2UIPV93M page 49 "图 1" / "图 2" (appendix X-ray figures) were removed because page 16/17 "图 1" / "图 2" (body figures with legend_like style) had higher dedup priority.

  • Root cause: dedup assumed all legends sharing the same (namespace, figure_number) key are duplicates of the same figure. It did not check whether the caption text itself is different.

  • Fix: added _strip_caption_number_prefix and _normalized_caption_body helpers. When a dedup collision occurs, compare the normalized caption bodies (figure-number prefix stripped). If bodies differ AND neither legend is a bundle-source duplicate, mark the lower-priority legend as same_number_distinct_caption_text and retain it — using a distinct key so both survive. Bundle-source legends always dedup (their text may differ but they are intentionally weaker duplicates).

  • Result: 2UIPV93M now has 19 matched figures (was 17). Page 49 Fig 1 / Fig 2 now correctly retained in figure_legends and matched to their X-ray assets. same_number_distinct_legends audit surface added to inventory. Bundle-source dedup (DWQQK2YB pattern) unaffected.

12.38 Reference items sorted by parsed number in tail reordering (2026-06-23)

  • Problem: 6FGDBFQN reference section showed order 1, 10, 2, 3, ..., 9 — reference 10 appeared between 1 and 2.
  • Root cause: fulltext.md is produced by render_fulltext_markdown_order_tail_blocks_reorder_tail_run_sort_blocks_by_column, which sorts by column/y-position (physical page layout). Reference 10's bbox was on the left column with y between ref 1 and ref 2. The existing sort_reference_blocks (number-based sort in ocr.py) only applies to the per-page OCR rendering path, not the main fulltext.md path.
  • Fix: added _ref_number_sort_key(block) helper that extracts leading reference number from block text (matches ^\s*(\d+)[\.\)]). Applied .sort(key=...) in three places within ocr_render.py: (1) _reorder_tail_run main path — on ref_section["bodies"] before emission; (2) _reorder_tail_run skip_section_grouping path — on refs list; (3) _order_tail_blocks fallback path — on ref_items only (ref_headings separated to avoid alphabetic mixing). All heading vs item separation preserved.
  • Result: references now emit in correct numeric order (1, 2, 3, ..., 10). Verified on 6FGDBFQN after rebuild.
  • Tests: 104/104 render tests pass (1 pre-existing failure in backmatter heading style, unaffected).
  • Commit: d96d803 on ocr-v2.

12.39 Merge-gate figure guardrail tests (2026-06-24)

  • Problem: the branch had landed the 6FGDBFQN sidecar fix and the 2UIPV93M same-number-distinct dedup expansion, but the test surface still left two merge-boundary risks weakly specified: (1) minor OCR drift in duplicate captions can be misinterpreted as a new same-number figure because _normalized_caption_body currently does exact string comparison after whitespace collapse only; (2) the strong table_like veto protects figure/table separation but had no reverse test proving that OCR table labels cannot suppress a real figure grid when figure evidence is otherwise decisive. The existing suite also did not explicitly prove that retained same-number-distinct legends stay accounted for through completeness + reader surfaces.
  • Root cause: prior tests validated positive 6FGDBFQN-sidecar behavior, bundle-source duplicate losers, and the existence of same_number_distinct_legends, but they did not pin the release boundary between acceptable retention and over-retention, nor the boundary between conservative table veto and false-negative figure loss.
  • Fix: added four focused synthetic tests in tests/test_ocr_figures.py: (1) test_same_number_distinct_legends_can_both_match_separate_assets locks the intended 2UIPV93M-style behavior when two captions share a figure number but own distinct assets; (2) test_same_number_distinct_legend_is_accounted_not_dropped verifies the retained distinct caption remains visible to completeness and reader consumption rather than disappearing as a gap; (3) test_same_number_ocr_minor_caption_variant_still_deduped is an intentional merge-gate red test documenting that punctuation-only OCR drift should still dedup; (4) test_table_labeled_img_figure_grids_still_separate_by_caption_when_caption_is_figure is an intentional merge-gate red test documenting that OCR table_like hints must not collapse multiple figure captions onto the same image grid.
  • Result: the suite now separates green regression locks from explicit red merge-gate blockers. No production OCR logic changed in this step.
  • Tests: targeted dedup/accounting tests pass locally; the two new merge-gate red tests are expected to fail until the remaining dedup-similarity and table-veto boundary decisions are implemented.

12.40 Merge-gate closeout: dedup normalization + fallback spatial scoring (2026-06-24)

  • Problem: two merge-gate blocker tests remained red: (1) test_same_number_ocr_minor_caption_variant_still_deduped — punctuation-only OCR drift (trailing period) surfaced as a false same_number_distinct_legends; (2) test_table_labeled_img_figure_grids_still_separate_by_caption_when_caption_is_figure — Figure 5 on a table-like multi-grid page was assigned the top grid instead of its nearer bottom grid.
  • Root cause (dedup): _normalized_caption_body used " ".join(body.lower().split()) which preserves punctuation; trailing . on one caption made "the graft." != "the graft".
  • Root cause (grid collapse): the group-aware sequential fallback (_score_legend_to_group) took the first same-page group scoring >= 0.5 (break), not the best-scoring one. With both caption groups table-vetoed and Figure 5 processed second, the top grid (first in y-order) always won even though Figure 5's spatially nearer group was the bottom grid.
  • Fix (dedup): changed _normalized_caption_body to re.sub(r"[\s\W_]+", " ", body).strip() after lowering, stripping punctuation/whitespace runs so that punctuation-only OCR drift normalizes to the same string.
  • Fix (grid collapse): replaced break-on-first-match with collecting all qualifying scores into a list, then picking the group with the highest score. Since _score_legend_to_group already incorporates spatial proximity, the bottom grid scores higher for Figure 5 and is correctly selected.
  • Result: both merge-gate blocker tests now pass. All 11 table-like guardrails remain green. All 4 distinct-caption guardrails remain green.
  • Regressions checked: SAN9AYVR same-number-narrative + formal-caption surface (2 tests) pass. Full bounded figure stack (225 tests) passes. Real-paper regressions: 7 passed, 5 skipped (fixtures absent), 0 failed.

12.42 OCR frontmatter-side zone hardening (2026-06-25)

Problem: Real page-2 body headings and body continuations could be swallowed into frontmatter_side_zone, then suppressed by the structural gate.

Root cause: _is_frontmatter_side_candidate() over-trusted early-page narrow/side-column geometry, _demote_early_frontmatter_body_leaks() still used broad page <= 2 assumptions, and accepted heading membership used page-local ids that could collide across pages.

Fix: Narrowed frontmatter-side entry to explicit furniture plus supporting geometry, added heading/body-continuation vetoes, restricted early demotion to the first surviving page before body-start, and normalized accepted heading membership ids to artifact-safe identities.

Result: 49PY5UCJ page-2 section heading survives as body heading; preproof-drop frontmatter behavior remains intact; heading verification no longer varies because of cross-page id reuse.

  • Problem: the merge-gate closeout changed _normalized_caption_body to strip all punctuation/whitespace runs via re.sub(r"[\s\W_]+", " ", body). That fixed trailing-period OCR drift, but it also widened dedup aggressively enough to collapse captions whose internal punctuation can carry meaning.
  • Root cause: the normalization treated internal punctuation and terminal punctuation as equivalent noise. This is too broad for same-number distinct-caption arbitration: IL-1 and IL 1, or other hyphenated token boundaries, can become the same normalized body even when they should remain distinct.
  • Fix: narrowed _normalized_caption_body to the smallest behavior needed by the blocker test: lowercase, trim, strip only terminal punctuation ([.!?:;,]+$), then collapse whitespace. This keeps the trailing-period OCR drift fix while preserving meaningful internal punctuation.
  • Result: the original merge-gate dedup blocker still passes, and a new synthetic guardrail now proves that internal punctuation differences can still survive as distinct same-number captions.

Problem: Two separate paragraph_title blocks with a close vertical gap were merged into one, garbling "THE MOLECULAR IDENTITY AND REGULATION OF THE MCU COMPLEX" with "The Pore Forming Subunits". Additionally, the merged heading got wrong level, single-word all-caps headings like "MCUb" and "EMRE" were assigned section_heading (H2) instead of sub_subsection_heading (H4), and OCR-internal newlines within a single heading block remained unnormalized.

Root cause: _merge_adjacent_headings had no case-consistency guard (all-caps vs title-case = different headings). _infer_heading_level's all-caps check used word_count >= 1, catching single-word sub-subsection headings. No normalization of OCR \n within block text.

Fix: Added case-consistency check to _merge_adjacent_headings (all-caps heading followed by title-case heading = separate, not wrap). Changed word_count >= 1 to word_count >= 2 in all-caps section_heading rule so single-word headings fall through to sub_subsection_heading. Added \n normalization for all paragraph_title blocks at merge entry.

Result: Heading hierarchy now correct:

12.44 Render-time heading level from font size, bold, and case style (2026-06-25)

Problem: Heading level assignment relied on text heuristics (uppercase ratio, word count) in _infer_heading_level because role_input didn't include span_signature, causing fallback to text-based level inference. The render-time block_heading_prefix was overridden by role-based prefix (section_heading## regardless of visual size). This gave ## MCUb / ## EMRE and could never infer #### Mitochondrial Calcium Uniporter correctly.

Root cause: Two layers of disconnected logic: _infer_heading_level had no access to font data; render-time had font data but used role-based override, not font-based prefix. Also bold signal in span_signature.bold was always False for many papers (field not populated by span extraction), so the real bold signal came from font-face name containing -Bd/-Bold suffix.

Fix: Replaced the entire heading prefix computation in render_fulltext_markdown to:

  1. Group heading blocks by font size bucket (span_metadata.size, rounded to nearest 0.5)
  2. Within each size bucket, subdivide by bold (detected via font-face suffix -Bd/-Bold)
  3. Within each size+bold group, subdivide by uppercase ratio > 0.7
  4. Assign prefixes sequentially (######### → ...)
  5. Removed the role-based override entirely — all heading roles use the same prefix map
  6. Used stable (page, block_id) keyed prefix map instead of id(block) to survive ordered_blocks reordering

Result: Heading hierarchy is now determined by actual visual signals. For 49PY5UCJ: ## THE MOLECULAR IDENTITY..., ### The Pore Forming Subunits, #### Mitochondrial Calcium Uniporter, #### MCUb, #### EMRE, ### The Regulatory Subunits.

Tests: python -m pytest tests/test_ocr_render.py -v --tb=short -q → 8/8 passed. python -m pytest tests/test_ocr_document.py tests/test_ocr_real_paper_regressions.py -x --tb=short -q → all passed.

12.45 page_assets safety gate regression tests (2026-06-26)

Problem: The page_assets group type in _score_legend_to_group had no dedicated regression coverage for its six internal gates: missing page context, compact geometry, multiple numbered legends, text separators, excessive coverage, and table-like assets.

Fix: Added 7 tests to tests/test_ocr_figures.py:

  • 6 unit tests directly invoking _score_legend_to_group covering each gate:
    • test_page_assets_missing_page_context_rejected — no page_blocks → rejected
    • test_page_assets_single_legend_compact_group_matched — single legend, compact 3-block cluster → matched
    • test_page_assets_multiple_numbered_legends_rejectedpage_numbered_legend_count=2 → rejected
    • test_page_assets_text_separator_rejected — body text between 2x2 grid blocks → rejected
    • test_page_assets_large_coverage_rejected — cluster covers >75% page → rejected
    • test_page_assets_table_like_asset_rejected — media group contains table-labeled block → rejected
  • 1 integration test: test_build_figure_inventory_no_page_assets_on_multi_legend_page — verifies build_figure_inventory does not emit page_assets_safe_gate evidence when a page has competing captions

Result: All 216 tests in test_ocr_figures.py pass (209 pre-existing + 7 new).

Remaining Known Issues

Three P0 gaps identified during the heading-level architectural analysis (not yet fixed):

  1. Footnote missing from _SKIPPED_BODY_ROLES — footnotes on body pages are not filtered from render output. Should add "footnote" to the skipped set so footnotes only appear when the render code explicitly handles them.

  2. Short "Table N" caption matching absent — very short caption text like "Table 1." has no dedicated matching path in the table ownership pipeline, causing some bare table labels to miss their table_asset.

  3. Table caption fallback ### should be ** — when a table_caption block has no table id and no table asset to embed, the render code currently emits ### {text}, which creates a spurious heading-level entry in the reading hierarchy. Should use **{text}** (bold) instead, matching the structured_insert pattern for text-level emphasis.


13. 2026-06-26: OCR Rebuild Production Run + Post-Rebuild Audit

13.1 What was done

Full ocr rebuild --all (699 papers) with --resume checkpoint support. Built and committed: progress bar fix (ASCII tqdm for Win), checkpoint persistence, --resume flag for interrupted rebuilds. Removed rogue [DEBUG] print in ocr_render.py:1295.

13.2 Audit findings

P1 — N6XCZD25: Body paragraphs on pages 3+ misclassified as structured_insert

Root cause: Physical Therapy journal PDF's two-column body pages have subtle layout boundaries (header/footer rules, column gutters, text column background) that _extract_visual_container_rects detects as container fill regions. All 56 body blocks post-page-2 get _in_visual_container=True, which feeds into score_structured_insert (line 176, ocr_scores.py): +0.3 container flag + 0.15 narrow_width = 0.45 ≥ 0.4 threshold → structured_insert_candidate → cluster-promoted to structured_insert.

The body_zone detection is correct. Zone assignment is fine. The bug is solely in score_structured_insert not checking body_spine_match (which would subtract 0.25 and keep the score below threshold).

Fix: When block is in body_zone with body-family font match, set body_spine_match=True in the structured_insert scoring call. This offsets the container flag.

P1 — Chinese Windows encoding: Non-ASCII PDF filenames get garbled in meta.json

Root cause chain:

  1. Zotero downloads a PDF with non-ASCII filename (e.g., Ongaro è - 2014.pdf) on Chinese Windows (codepage=936/GBK)
  2. The UTF-8 bytes \xC3\xA8 (è) are interpreted as GBK bytes → produce U+7B49 (等) instead
  3. This damaged filename is stored on NTFS
  4. pdf_resolver.py:resolve_pdf_path constructs a path using the BBT export's correct UTF-8 name, which doesn't match the garbled NTFS filename → is_valid_pdf() returns False
  5. No glob fallback exists in resolve_pdf_path → PDF path is lost
  6. OCR queue falls back to filesystem glob → finds the garbled filename → writes it to meta.json

Affected papers: PUP9JRFH (Chinese title garbled), 9EQV3ABQ, AA6P3IE5, N6XCZD25, U746UJ7G (Latin-accented filenames garbled)

Fix (two layers):

  1. Add glob fallback in resolve_pdf_path: when storage:KEY/filename exact match fails, glob storage/KEY/*.pdf to find the actual file regardless of filename encoding
  2. For already-corrupted meta.json: ocr rebuild --all + the glob fix will auto-correct source_pdf paths on rebuild

13.3 Issues logged (deferred)

8LZUYXMH — Wrong PDF bound (Supplementary Material instead of main article)

Fixed: ocr redo with correct PDF at storage/HMN8IGBW. Rebuild completed.

SWDN9RHF — Short paper (2-page letter) health falsely red

Content and fulltext correct. Health score penalizes for no headings (heading_total: 0) and no abstract — standard Letter to the Editor format. Defer to health model enhancement for page_count <= 3 short papers.

VVIT8SVK — Accepted Manuscript / Pre-proof layout

37-page paper with Accepted Manuscript cover page. Pipeline has no preproof_cover_zone or non_body_insert template for this format. Defer to layout template expansion.

14.1 Figure Number Inference for Leading Gap

Problem: OCR lost "Figure 1." text in N6XCZD25 (rendered inside figure image). figure_inventory produced figure_unknown_005.

Root cause: No inference logic to fill figure_number: None for matched main-sequence figures when the missing number can be deduced from sequence gaps.

Fix: Added _infer_missing_main_figure_numbers() in ocr_figures.py — 8-step algorithm:

  1. Build known set from matched_figures with int figure_number (main namespace only)
  2. Build eligible unknown set (must have legend/asset block IDs, valid settlement_type, no frontmatter veto, resolvable bbox)
  3. Skip reasons: no_eligible_unknowns, known_min_not_2, multiple_eligible_unknowns, first_known_not_found, first_known_bbox_unresolvable, unknown_not_before_first_known, intervening_unknown_unorderable, intervening_items_between
  4. Order check via figure_order_key = (asset_pages_min, legend_page, y0, x0)
  5. Noise check across matched/held/ambiguous figures
  6. Mutation: figure_number=1, figure_id=figure_001, number_inference metadata on item + inferred_figure_number on figure_legends

Helpers added: _FRONTMATTER_VISUAL_VETO, _has_frontmatter_visual_veto(), _FIGURE_MARKER_PATTERN regex, _extract_figure_marker(), _coerce_int_figure_number(), _resolve_legend_bbox() (3-tier fallback).

First release scope: Single leading [1] gap only. Middle gaps, multiple leading gaps, end gaps deferred.

Bug found during rebuild validation: legend_block_id stored as int (2) in JSON, not string. Eligibility check isinstance(legend_block_id, str) rejected it. Fixed by coercing to str().

Result: N6XCZD25 rebuild → figure_number_inference: {status: "accepted"}, figure_001 in matched_figures, 0 figure_unknown in fulltext.

14.2 Evidence-Driven Container Admission Rewrite

Problem: Blue sidebar box (156x43pt) excluded by old height<50 gate; page frame rects and thin gray decor rects falsely admitted as containers, causing [!NOTE] false positives.

Root cause: Size-first gates (width/height thresholds) are not selective enough. Real callout boxes can be short; fake containers can be large.

Fix: Replaced absolute size gates with evidence-driven admission in _extract_visual_container_rects():

  1. Feature extraction per drawing (fill, border, gray, line-like, page-sized flags)
  2. Page-sized/crop-like: completely excluded (no standalone, no grouping)
  3. Line-like: grouping pool only, not standalone
  4. Vertical component merge (x_overlap >= 0.8, gap -2..5pt)
  5. Merged rect can replace child candidates (not additive)
  6. Text evidence required (PDF text + OCR raw blocks via _map_ocr_bbox_to_pdf_rect)
  7. Accepted rects sorted by area descending

Call site rewritten: Per-page three-phase loop (clear flags → compute containers → mark blocks) replaces old lazy-cache approach.

Spec/plan review round 3: 4 must-fix changes applied:

  1. _has_container_text() gets pdf_page param for real coordinate mapping
  2. Three-phase loop (not incremental cache)
  3. Page-sized features excluded from grouping_pool
  4. Merged rects replace children + area-descending sort

Result: 27 tests pass (11 new + 16 existing). N6XCZD25 blue box detected as _in_visual_container=True.

14.3 Blue Box Not Rendered as Callout

Problem: After container admission, blue box was detected but NOT rendered as > [!NOTE] in fulltext.

Root causes (3 bugs):

  1. score_structured_insert weight for _in_visual_container was 0.3 (from old unreliable gates). Blue box scored 0.45 → stuck at structured_insert_candidate.
  2. _container_text was set to True (bool) by the three-phase loop — renderer expected string, crashed.
  3. Version bump was missing for the second backfill code change, so rebuild skipped backfill.

Fixes:

  • Bumped _in_visual_container weight 0.3→0.45 (evidence-driven is reliable)
  • Added sidebar_header_phrase check (+0.2) for "available with this article" etc.
  • Normalize newlines in text before keyword matching
  • _container_text set to block.get("text", "") instead of True
  • Renderer guard: isinstance(container_text, str) check
  • Bumped CURRENT_SPAN_VISUAL_CONTAINER_VERSION to force backfill re-run

Result: Blue box score now 0.80 (visual_container 0.45 + sidebar_header 0.2 + narrow_width 0.15) → structured_insert. Fulltext renders:

18.3 Fix 1: Figure-Internal Text Containment (P1 → P1 ready)

Status: Spec finalized 2026-06-27 — §3.2/§3.3 updated to match §3.7 contracts. Plan-ready. Spec: docs/superpowers/specs/2026-06-27-figure-containment-and-backmatter-boundary-design.md §3

Key design decisions in spec:

  • A: Call site after build_figure_inventory, before render_fulltext_markdown — NOT inside normalize_document_structure
  • B: Fallback asset eligibility includes media_asset with figure-like family, not only figure_asset
  • C: Fallback assets exclude those already covered by matched_figures.asset_block_ids + dedup with _highly_overlaps_any_matched_region
  • D: Proximity clustering (_cluster_bboxes_by_proximity, margin=40), not strict overlap
  • E: Containment = center-inside + overlap/block_area >= 0.85 + width ≤ region_width*0.95

Render behavior: Renderer skips figure_inner_text by role. Do not mutate render_default/index_default in first release.

Previously recorded: Location: ocr_roles.py:78 (_PANEL_LABEL_PATTERN), ocr_roles.py:221 (_looks_like_figure_inner_label) Root cause: Three gaps:

  • Gap A: Figure 1 caption demoted → no matched_figures entry → no cluster_bbox for containment check
  • Gap B: Inner-text detection only fires for raw_label=="text" — figure-internal blocks have vision_footnote/paragraph_title/figure_title raw_labels
  • Gap C: No spatial containment logic anywhere (_bbox_contains exists in ocr.py but is never used for figure_inner_text)
  • Gap D: vision_footnote blocks have no inner-figure pathway

Existing utilities:

  • _PANEL_LABEL_PATTERN — matches A, (B), C. (single-letter only)
  • _looks_like_figure_inner_label — 12-char limit, 80px proximity to image/chart
  • _bbox_contains(outer, inner, margin=12) — in ocr.py:833, unused for this purpose
  • media_clusters() / _cluster_bbox() — compute union bbox of asset groups

Fix proposal: Add render-hygiene pass after build_figure_inventory: for each matched_figures entry, use cluster_bbox to check all text blocks inside it. Assign figure_inner_text to any non-figure-role block fully contained. Need to handle composite figures with caption demoted upstream.

18.4 Fix 2: Reference Sorting Bracket Gap (P0)

Root cause: _ref_number_sort_key (ocr_render.py:481) regex r"^\s*(\d+)[\.\)]" doesn't match [N] bracket format. References like [10] fall through to return (1, text) — full-text lexicographic sort → [10]...[19][20]...[29][2]...[9].

Why multi-column fix didn't catch it: That fix targeted page/section ordering, not within-reference numbering. _ref_number_sort_key was used as the fallback sort with wrong key function.

Call chain:

  1. render_fulltext_markdown()_order_tail_blocks()_reorder_tail_run()
  2. skip_section_grouping path: refs.sort(key=_ref_number_sort_key) (line 524)
  3. Main path: ref_section["bodies"].sort(key=...) (line 672)
  4. Fallback: ref_items.sort(key=...) (line 864)

Existing correct function: parse_reference_number() (ocr.py:607) handles both N. and [N] formats — but ocr_render.py never imports from ocr.py. They are independent modules.

Fix: Add second capture group to regex: r"^\s*(?:(\d+)[\.\)]|\[(\d+)\])" — use m.lastindex to pick the right group. Two capture groups avoids false matches on plain year numbers.

18.5 Fix 3: Backmatter Boundary Recognition (P1 → needs redesign)

Status: Spec reviewed 2026-06-27 — NOT ready for implementation. 3 blockers unresolved:

  1. Multi-page refs: reference_headingreference_zone.end. Algorithm must use verified reference zone extent, not the heading block alone.
  2. Same-page body+ref: NC66N4Q3 page 56 column-mixed body/ref needs column-aware partition, not page/y-only.
  3. Pre-ref known-heading gate: Exact-match only, document-progress >= 0.50 guard, no substring matching.

Fix 3 is deferred until a new spec resolves all 3 blockers. See spec §4.8 for full review.

Root cause (25K5KZAQ): Three converging failures:

A. Pattern gap in heading classification:

  • _is_backmatter_boundary_heading() (ocr_roles.py:303) checks has_container_words: "ADDITIONAL" in upper or "DECLARATION" in upper or "INFORMATION" in upper
  • "CRediT authorship contribution statement" contains none → returns False
  • is_visually_heading requires font_size >= 11pt; CRediT blocks at 7.97pt bold → False
  • Both checks fail → subsection_heading instead of backmatter_boundary_heading

B. Multi-column layout masks body-end:

  • Page 10: left column has Discussion body only, right column has CRediT+Ethics+Declaration
  • _detect_forward_body_end() checks per column: left has any_body_without_tail=True → scan continues
  • Declaration anchor below CRediT/Ethics → backmatter_start after CRediT

C. Normalization skips pre-boundary blocks:

  • _normalize_backmatter_roles_after_boundary() line 2062: unconditionally skips subsection_heading
  • Line 2103: blocks before backmatter_started are skipped
  • _promote_tail_body_candidates() requires anchor above or page > spread_start — CRediT is above anchor, same page as spread_start

Affected papers: All papers with CRediT/Ethics/Declaration backmatter headings that use small font (<11pt) and lack container keywords.

18.6 Fix 4: Figure Caption → non_body_insert (P0)

Root cause (YGH7VEX6 Figure 2): _detect_non_body_insert_clusters() (ocr_document.py:3792) includes figure_caption in _INSERT_CANDIDATE_ROLES. Figure 2 caption (width 442px) passes the strong-spine-quality check: font_mismatch=True AND 442 < 0.9×507(median_width)=456.3. Same page has another candidate (idx 1, width 232) → cluster of 2 → both marked non_body_insert.

Downstream effect: Figure 2 caption never enters figure_legends → asset (p2:14) consumed by Figure 1 → Figure 2 invisible to entire inventory system.

Fix: Remove figure_caption and figure_caption_candidate from _INSERT_CANDIDATE_ROLES (line 3846). Original rationale ("PaddleOCR labels narrow author bios as figure_title/figure_caption") is a corner case handled by existing family-profile rescue.

18.7 Fix 5: Demoted Body Paragraph → Figure Legend (P0)

Root cause (YGH7VEX6 Figure 11): _is_validation_first_legend_candidate (ocr_figures.py:405) checks:

  • zone == "display_zone" — True for figure-mention body text
  • style_family == "legend_like" — True (contains "Figure N")
  • marker_type == "figure_number" — True

These are proxy geometry/style signals that don't reflect the pipeline's role decision. The block was already demoted to body_paragraph during candidate resolution. Despite demotion, it re-enters legend matching.

Figure 11 on page 6:

The body paragraph won the primary slot (figure_011 at 0.885). Real caption (figure_s011 at 1.0) was demoted to supplementary. Render uses wrong legend text.

Matching evidence: ["same_page", "adjacent_x", "anchor_supported", "caption_text_supported", "family_supported", "multi_asset_coherence_bonus"] — NOTICE: no "caption_above_or_below" or "nearby_y". The match was cross-column via horizontal adjacency, which is plausible geometry but wrong semantics.

Fix: Filter demoted body_paragraph from figure_legends before _is_validation_first_legend_candidate check. Do NOT mutate render_default/index_default. Write full audit trail to rejected_legends (page, block_id, text, role, seed_role, rejection_reason).
