Handoffs

Session "you are here" state goes in docs/handoffs/HANDOFF.md (a single, canonical, overwritten-each-time file — git is the dated archive). That file is owned by the creating-handoffs skill; this README just provisions the location. None exists yet — one appears when work is handed off mid-flight.
